Position Summary:

The Continuing Care Case Manager is responsible for working with the Senior Executive Director in partnership with the Executive Directors, Assistant Executive Directors, Health & Wellness Directors, and Leasing Teams to support residents as they need services throughout Balfour’s continuum of care. This is an outstanding opportunity for individuals who are team oriented, dedicated, and enthusiastic about working with seniors. In this rewarding role, the Case Manager will be responsible for supporting the residents and their families as they safely and successfully age in place and/or transition through the continuum of care.


As the Continuing Care Case Manager, you will:

  • Perform care continuum process that addresses, plans, coordinates, monitors and evaluates the options and services required to support the resident’s needs
  • Serve as a resource to residents and their families to educate and help navigate the Balfour continuum of care, as well as options to support care needs
  • Coordinate the care and services for residents identified as needing assistance or meeting care management support
  • Assess resident’s care needs, access to and/or use of community resources
  • Demonstrate critical thinking skills when utilizing care continuum process
  • Schedule and lead care plan meetings with residents and family members
  • Work with new or transferring residents to complete the necessary documents and preparations to ensure a smooth transition to or within the community
  • Serve as an ongoing resource, responding to resident and family questions and needs


Qualifications:

  • Degree in social work or equivalent work experience
  • A talent for interacting with a wide variety of people, particularly the elderly, their family members and professional influencers, physicians, nursing, health personnel
  • Ability to work in a team environment but also able to work independently and organize workload
  • Effective verbal and written communication skills are required, including the ability to communicate openly, honestly and responsibly with others
  • Good organizational skills required
  • Knowledge of medical terminology
