Job Title: Community Support Assistant

Department: Mental Health Services

Scope of Service: Provides comprehensive community-based residential and support services to persons with mental health, intellectual, and addiction needs. Provides hands-on, intensive skill- building interventions and supports to enrich and achieve an overall level of independence and well- being. Collaborates with Community Support Specialists and program to support client in achieving aspirations through person-centered, strengths-based, and culturally competent methods. Performs other duties as assigned.

ESSENTIAL JOB FUNCTIONS

Client Care:

  • Establishes rapport and positive working relationships with clients while maintaining professional ethics and boundaries.
  • Enhances a client’s independent living skills by engaging, assessing, and providing various modalities of teaching and assistance (e.g. modeling, prompting, hands-on assistance).
  • Provides hands-on support and guidance for activities of daily living such as personal care/hygiene, housekeeping, laundry, food management/nutrition, budgeting, health and safety, leisure, and other activities which promote healthy living.
  • Provides transportation to and from appointments as identified.
  • Educates and trains clients with travel planning and use of public and private transportation.
  • Conducts regular apartment/room inspections and assists the clients in maintaining the cleanliness of their living spaces which may require hands-on assistance.

QUALIFICATIONS

  • Bachelor’s Degree preferred. High School Diploma or equivalent considered with a minimum of 3-years’ experience in related field.
  • Prior experience working with people with severe and persistent mental illness, intellectual disabilities, and/or addiction.
  • Valid Connecticut Driver’s license with clean driving record is required. Program positions may require the employee to maintain reliable transportation and be willing to transport clients in personal vehicles.
  • First Aid/CPR certification or willingness to obtain certification within 90 days of hire.Employee is responsible to maintain ongoing certification.
  • Proficient computer skills required, including Microsoft Office and electronic record keeping.

The following positions are available at various locations listed below:

Per Diem/Substitute Staff Rate: $17.25 - Weekend Differential of $18.25
FT/PT - First/Second/Third Shift: $17.75 - Weekend Differential of $18.75

North Colony – Meriden, CT:

FT 40 hours Tues/Wed/Thurs 7am-3pm Fri/Sat 6am-2pm

FT 40 hours Monday - Friday 1pm-9pm

FT 40 hours Tuesday - Saturday 11pm - 7am

PT 8 hours every other Friday 1pm-10pm

PT 16 hours Sat/Sun 1pm - 9pm

Hartford Ave– Wethersfield, CT

FT 40 hours Mon- Fri 8am-4p

FT 40 hours Sun-Thur 3pm-11pm

PT 16 hours Sat/ Sun 7am-3pm
