TITLE: Independent Living Coordinator/Community Transition

DESCRIPTION / SUMMARY:

Options is a 501c3 non-residential organization that empowers people with disabilities to live independent and productive lives in the community. Options cover 17 counties in Northeastern Wisconsin, providing advocacy, independent living skills training, information, referral, peer support, and transition services for people with disabilities.

Independent Living Coordinator will assist consumers of Options with varying disabilities gain the skills, support, resources, and information they need to live independently in the community. This position will provide services in Options 17 county service area. The applicant can work remotely out of the Fond Du Lac or Oshkosh area.

QUALIFICATIONS:

Education: A minimum of two years of post-diploma work in human services, healthcare, or disability-related field is required.

Experience: Knowledge of human services programs affecting people with disabilities. Knowledge of disability rights and IL philosophy. Ability to develop and provide independent living skills training. Understanding different community resources and navigating various healthcare systems throughout Northeastern Wisconsin is preferred. Individuals with a disability are encouraged to apply.

License: Valid Driver’s license and insured vehicle or ability to arrange for independent travel. Daily travel is expected.

JOB RESPONSIBILITIES

Essential Functions: The Independent Living Coordinator will help individuals with disabilities:

· Receive reliable information and resources.

· Consult with consumers in a congregate setting to identify strategies to relocate to community living situations through assistive technology, referral to appropriate agencies, independent living skills training, and other appropriate referrals as necessary.

· Serve as point of contact for partnered agencies such as hospitals and nursing homes.

· Learn life skills such as ADL training, financial management, conflict resolution, and organization skills by facilitating one-on-one or group workshops.

· Cope and overcome the challenges of living with a disability by providing nonclinical emotional and peer support.

· Advocate individually by assisting with issues regarding housing, employment accommodations, transportation, etc., and advocating for systemic change.

· Navigate through complex bureaucracies and systems to receive the services they need.

· Facilitate transitioning to higher education, employment, or other independent living situations.

· Selecting Assistive Technology to help overcome independent living barriers and provide ongoing equipment training.

· Provide community activity services in Options’ service area, including disability-related presentations, booths at area disability-related fairs/events, etc.

· Represent the interests of people with disabilities by serving as members of area committees and boards, e.g., C-Cots, Transportation, Housing, etc.

· Record all data as required.

· Practice and promote the Independent Living philosophy in all aspects of this position.

SALARY: Dependent on experience and education.

40 hours per week, generally 8:00 AM – 4:30 PM; however, occasional evening work may be required.
