Job description
Northern Counties Health Care, Inc is looking for a full time (32 hrs) Community Health Worker (CRC) to serve the Island Pond/Newport area. The Community Health Worker will assist in the coordination of and navigation of services to patients and their families with medical, dental, social, and mental health needs. This position requires the CRC to become a Certified Vermont Health Connect Assister; includes interacting with patients in a Primary Care setting and in the community. The CRC will be responsible for helping patients and their families navigate and access community supports and services as well as assist in applying for state and federal programs.This position has a unique "in-house" Wellness Program that the CRC will work closely with the staff to promote and schedule. An associate's degree is required. Six months to one year experience in human services field is also preferred. Ability to work independently, present in a professional, confident manner and excellent documentation skills necessary.
