Community Health Worker - We are seeking an experienced, dynamic, and professional Community health worker to join our team! The ideal candidate will have a strong background in community health. The role's key focus is to provide short-term coordination to community members, connecting them with social, health, and wellness services they need access to.

Responsibilities

• Assist clients in their homes, community, or clinic setting. Communicates to clients/patients the purposes of the program and the impact or improvement it may have on their wellbeing.
• Help patients identify socio-economic issues that affect their health and develop health/social management plans and goals to assist them.
• Document all client encounters and contracts made on behalf of clients.
• Complete and submit monthly reports; maintain accurate electronic and hard copy client files, which include notes, the release of information, assessments, and other medical documents.
• Document activities, care and service plans, and outcomes or achieve goals achieved by the client in an effective manner.
• Educate clients on how to properly use the Emergency Room and provide information for viable alternatives.
• Coach patients in the management of their chronic health conditions and self-care.
• Assist patients in understanding care plans and instructions given by medical professionals. Motivate and encourage patients/clients to be active and engaged participants in their health and wellbeing.
• Assist clients in accessing health-related services, including obtaining a medical home, providing instruction on using the medical home appropriately, and overcoming barriers to obtaining needed medical care and /or social services.

Job Types: Full-time, Contract

Pay: $20.00 - $25.00 per hour
