SUMMARY:

Community Health Representative duties shall be under the direction and guidance of the PHN Supervisor. This is considered a Category I position for potential exposure to Bloodborne Pathogens and Hazardous Materials under OSHA.

MAJOR DUTIES AND RESPONSIBILITIES:

· HHA Certification within one (1) year of employment

· Accomplish a minimum of 50 home visits per month

· Charting on at least 10 ongoing individual charts monthly.

· End of Month reports, mileage logs and auto inspection forms are due on the 3rd day of the moth to immediate supervisor.

· Accurately measures and records TPR and blood pressure on each home visit. Blood sugar when applicable.

· Helps patient with ADL. (Bathing, brushing teeth, shampoo/combing hair, amblation.)

· Assist patient with grocery shopping, laundry, bed making as assigned.

· Transport patient according to priorities outlined in Department Manual.

· Provides basic health teaching in home and community to include but not limited to the following topics; diabetes, newborn care, immunization, FAS, Birth control, These teachings will be monitored by PHN and documented on medical records.

· Attend at least 10-In-Service meetings per year.

· Does not compromise patient confidentiality.

· Quick to volunteer and assist in special projects as assigned by supervisor.

· Identify special problems in the home or community and notifies appropriate health team. member. Attends a minimum of one (1) outreach patient care conference per month.

· Reports to work promptly at scheduled time and uses sick time per organizational policies.

· This position may require working on the weekends, early and late unscheduled hours to meet the health needs of patients.

· Additional duties as assigned.

MINIMUM QUALIFICATIONS:

Education: High school diploma or equivalent.

Experience: One year experience in health field and/or dealing with the

public preferred. Formal training as a community health worker preferred.

Skill to: Must have clear and effective oral communication skills.

Work in the Indian population in the service area and gain their confidence and acceptance; take instructions; follow directions and maintain records. Read and write at level for satisfactory job performance. Work a variable work schedule. Must demonstrate auto and patient safety; and must provide a Department Motor Vehicle (DMV) printout before hired on and then bi-annually.

Ability to: Maintain strict confidentiality procedures. Work with other staff members in a team effort. Must be able to work with the Indian Community and be sensitive to the Indian culture and its needs. Must have good physical and mental health and demonstrate behavior appropriate to position. Must be free of communicable diseases, maintain personal cleanliness. Physical will be due annually. Must be able to develop and maintain good working relationships with the medical professionals and para-professionals.

Work Environment: This assignment will be carried out in an office setting, vehicle, or home environment with potential for frequent exposure to contagious diseases. There is infrequent exposure to unstable and/or hostile patients.

License: Must possess a valid California driver’s license and safe driving

record. Must be insurable by the corporation’s insurance carrier. A current DMV printout will be required every six months.

Physical Demands: Required to drive over hazardous road conditions if the need

arises. Required to assist patients by lifting, supporting guiding to and from vehicles and buildings; must be able to push patient in wheelchair.

Required to lift, load, and transport equipment or other corporate items. Provides courier service to patients/clinic. Required to sit for long periods of time either in vehicle or office setting.
