POSITION SUMMARY
The Community Health Outreach Coordinator engages the community in the services and activities of the Health & Wellness Center and provides education, outreach and support to the community as well as individuals and families served. Wheeler Health & Wellness provides an integrated, comprehensive continuum of patient-centered primary care, behavioral health and community resources, supported by multi-disciplinary teams to reduce disparities in care, enhance health equity and achieve the triple aim of health reform: improve health, reduce costs & increase quality.

EDUCATION AND EXPERIENCE/QUALIFICATIONS
BA/BS degree plus 3 years related experience or MA/MS degree plus 1 year related experience in community organizing, marketing, prevention/health promotion or other social service field is required. Education may be substituted with experience on a year for year basis. A valid Connecticut Driver’s license is required, may function as a member of the Mobile Medical Team, as assigned.

Ability to work on a team to ensure program goals are met, communicate effectively verbally and in writing, and is committed to personal and program growth. Ability to manage time effectively. Ability to represent the Clinic in a professional manner.

ESSENTIAL DUTIES AND RESPONSIBILITIES
Participates in the planning, coordination, and implementation of community engagement, education, outreach and support activities designed to enhance access to primary and behavioral health care. Outreach and education activities include prevention, health promotion, and wellness training events and programs; health screening events; recovery support services; mutual support groups; and referrals to other community resources.

Coordinates and facilitates community training events and programs as appropriate for all health center locations.

Strengthens existing collaborations and establishes new partnerships and patient referral sources in the community. This may include, but is not limited to, schools, senior centers, food pantries, the faith-based community and other human services and community organizations.

Plans and participates in community events, committees, planning initiatives and other activities to enhance community and individual health outcomes. Represents Wheeler Health & Wellness in the community as needed.

Coordinates access to resources, including referral to specialty services and assistance in accessing insurance coverage and pharmacy patient assistance programs, and advocates for individuals and families as needed.

Maintains records of coordination of care, outreach, patient support and/or case management activities for reporting and tracking purposes and completes all documentation for client and program records in accordance with documentation timelines and in compliance with Medicaid, licensing and other regulatory requirements/agencies as defined by program.

Supports daily program specific operational functions as required. May transport clients to appointments or linkages as appropriate. May involve visiting clients in their homes and/or community settings

Attends and is prepared for scheduled supervision, team meetings, staff meetings or rounds. Seeks additional supervision or consultation as needed and follows through with supervisory directives.

Demonstrates clear, concise, professional writing and oral reporting skills.

Works effectively as a member of a multi-disciplinary team by communicating appropriately and positively with colleagues, families, internal and external service providers, administrative staff and actively preparing for and participating in team meetings.

Maintains competencies to meet expectations in all position-specific key performance indicators.

Continues to develop knowledge and understanding about the history, traditions, values, family systems, and artistic expression of groups served as well as uses appropriate methodological approaches, skills, and techniques that reflect an understanding of culture.
