Position Overview:

Serves as the liaison between Sutter Health and appropriate community collaboratives for the designated operating unit. Facilitates all initiatives to address community health needs as identified in the Community Needs Assessment. Manages a portfolio of community initiatives and interfaces with community leaders on a regular basis.

Key to this position will be a self-directed professional who can work autonomously and can creates meaningful community benefit program investments.

JOB ACCOUNTABILITIES:

ENHANCE AND STRENGTHEN COMMUNITY BENEFIT ACTIVITY & PRESENCE IN THE COMMUNITY.
  • Serves as a representative of SH at various local and county community health events, coalitions, taskforces, and collaboratives for a designated operating unit.
  • Manages a portfolio of community initiatives.
  • Interfaces with community leaders on a regular basis.
  • Collaborates on the development of strategic investments, programs and partnerships aimed to improve health and access to care for the uninsured and underinsured residents.
  • Plans and coordinates all onsite, and community based, community health related events.
  • Attends each event to ensure community engagement.
  • Arranges a post event coordination to ensure that all patients with abnormal screenings are followed up with and are provided with needed access to care resources in a timely manner.
  • Completes all essential contracts and agreements related to event logistics and accounts for all expenses for community benefit reporting.
  • Identifies appropriate stories highlighting community benefits or community relations with other community staff

DEVELOPS AND MAINTAINS RELATIONSHIPS WITH LOCAL COMMUNITY ORGANIZATIONS.
  • Establishes and maintains positive and effective working relations with governmental bodies, community based organizations, medical clinics, health and social service providers, and collaborative groups.
  • Actively initiates engagement with various groups to achieve CB goals and build community partnerships to address both social and economic disparities in the community.
  • Develops and maintains relationships with area-wide Community Based Organizations (CBOs) and their leadership through attendance at meetings and onsite visits
  • Identifies, through local community involvement, programs that could be considered as candidates for a broad collaborative Community Benefit activity.
  • Manages activation elements of community benefit sponsorship activities and community event participation.
  • Communicates Sutter position and commitment to community health/community benefit to internal and external audiences.

COMMUNITY RELATIONS REPORTING.
  • Prepares and reports all mandated and regulatory Sutter Health, state and federal reports related to hospital community benefit.
  • Collects and analyzes all necessary financials from Sutter Health Finance to confirm accuracy in reporting for assigned affiliates/geography.
  • Ensures detailed and accurate accounting of community benefit expenses and program outcomes for community benefit reporting and grants administration.
  • Works collaboratively with local and county stakeholders during Community Health Needs Assessment (CHNA) process and has the authority to review, analyze and monitor affiliate community benefit plans to assess relevancy based on the findings of the CHNA.
  • Develops and reports annual OSHPD plan updates and hospital Implementation plans to clearly describe the hospital’s strategic plan to address priority health needs as identified in the CHNA.
  • Manages reporting and tracking for local affiliate reporting requirements and system level initiatives/dashboard.
  • Coordinates quarterly and biannual impact reports, demonstrating the impact of all Community Benefit programs for leadership and other audiences.

COMMUNITY HEALTH PROGRAMATIC ADMINISTRATION.
  • Manages the Community Benefit Planning Process for Valley/Bay wide initiatives (as defined by geographic area position supports), and implements programs accordingly.
  • Oversees and streamlines the application process for all Community Benefit investment requests, organizes requests and is part of the review and approval process.
  • Tracks initiative budget and investments.
  • Manages reporting needs for initiative portfolio:
  • Tracks budgets and investments, government and community programming impact(s) and outcomes and grant program metrics
  • Works closely with Shared Services, Accounting and Finance, to process partner invoices and payments.
  • Identifies reporting improvements
  • Helps maintain a Community Benefit tracking device, outlining all approved and pending community benefit programs.
  • Collects and organizes all documentation related to each initiative
  • Coordinates all necessary BAAs, BSAs and MOUs
  • Authors memorandums of understandings

COMMUNICATIONS COORDINATION.
  • Works with other team members, Communications staff and outside partners to identify appropriate stories highlighting community benefits or community relations.
  • Communicates affiliate, area, and system-wide position and commitment to community health/community benefit to internal and external audiences.
  • Assists with drafting the Community Benefit Report to key audiences as needed.

EDUCATION:
Equivalent experience will be accepted in lieu of the required degree or diploma.

Bachelor's: Public Policy or a related field, or equivalent experience or equivalent education/experience

TYPICAL EXPERIENCE:

2 years recent relevant experience.


SKILLS AND KNOWLEDGE:


Ability to handle multiple projects and deadlines.

Strong attention to detail, proficient organizational skills and analytical thinker.

Ability to rapidly assimilate and synthesize information and thrive in an evolving environment.

Demonstrated ability to write comprehensive and accurate technical documents.

Demonstrated ability to work with financial reports, track financial data and evaluate metrics.

Knowledge of public policy, the political process, government and community affairs and event coordination. A significant understanding of community-based organizations would be an asset.

Knowledge of Microsoft Word, Excel, PowerPoint, and Outlook software.

Excellent interpersonal and communication skills both oral and written.

Demonstrated ability to work independently with minimal direction.

Pay Range: 30.98-46.47

The salary range for this role may vary above or below the posted range as determined by location. This range has not been adjusted for any specific geographic differential applicable by area where the position may be filled. Compensation takes into account several factors including but not limited to a candidate’s experience, education, skills, licensure and certifications, department equity, training and organizational needs. Base pay is just one piece of the total rewards program offered by Sutter Health. Eligible roles also qualify for a comprehensive benefits package.
