Job Responsibilities

Under general supervision, gathers, analyzes, and monitors data regarding communicable diseases within a health district. Ensures implementation of timely disease intervention strategies to decrease the prevalence of such diseases. Performs reporting and surveillance activities as well as conduct data collection and analysis. Prepares documentation of all communicable disease case investigations occurring within the district. Work closely with all HIV Medical care providers to ensure a smooth transition from HIV testing to HIV Medical care. Maintain a good working relationship with district case manager to assist with any barriers from entering HIV Medical care within 72 hours from HIV diagnosis. Work closely with district Ryan White staff to ensure optimal patient outcome. Build a network of community-based organizations for referrals to linkage to care.

Minimum Qualifications

Bachelor's degree from an accredited college or university AND six months of related experience OR Associate's degree from an accredited college or university AND Two years of experience in a related area OR Four years of experience in a medical related field OR licensure as a registered nurse AND One year of nursing experience OR One year of experience at the lower level Communicable Disease Spec 1 (HCP080) or position equivalent.

Additional Information

PREFERRED QUALIFICATIONS: In addition to the minimum qualifications, applicant must possess a strong knowledge of HIV/AIDS and STD’s. Experience in conducting outreach and disease intervention. Completions of ISTDI and or ASTDI and PassPort. Case Management skills and Phlebotomy are a plus
