Job description

Looking for 10+ full-time CNAs for our 7-3; 3-11; and 11-7 shifts in our Atlanta Center. We need you to be:

  • Knowledgeable of The Fountainview Center’s Mission, philosophy, service standards and strategic plan and can communicate them to other staff as well as clients and customers.
  • Familiar with the facility personnel policies, procedures and employment regulations.
  • Participate in the facility’s Continuous Quality Improvement Program.
  • Utilize the information in each resident’s life story and a flexible approach for meeting individual care needs, in order to provide person-centered care.
  • Have the ability to adapt services, programs and care as the abilities of the residents change, make suggestions to improve the quality of life for each resident.
  • Be able to accept change and make recommendations in reaching optimal life enhancing activities for the dementia resident.
  • Encourage resident self-care and functions as a “Best Friend” to each resident.
  • Notify the charge nurse of resident changes in meal intake, chewing or swallowing difficulties so appropriate modifications can be made.
  • Provide hydration and nourishment by offering /assisting with fluids, snacks, meals and supplements; record intake accurately.
  • Assure resident safety by maintaining safe environment, which includes room safety, chair or wheelchair safety, equipment and all aspects of resident care including the monitoring of the Roam Alert bracelets for residents deemed to be an elopement risk.
  • Meet the physical needs of the residents including transfers (including use of lifts), incontinence care, bathing grooming, oral care, dressing, food and nourishment, positioning, bathing and all aspects of care.
  • Be knowledgeable of the resident special needs during mealtime, adjusts environment to maximize benefit from the dining experience. Prepare tables for meals. Serves, and assists with meals and have the ability to recognize the any special needs of the resident for adaptive equipment for eating and/or the ability to appropriately cue the resident.
  • Carry out assignments given by the charge nurse and reports changes in the residents to him/her. Communicates constructive feedback regarding any appropriate changes in care technique or work assignments.
  • Communicate with the charge nurse and CNAs leaving their shift to ensure continuity of care relating to resident changes/condition/needs.
  • Utilize equipment in an appropriate manner and secures equipment when tasks are completed. Report any equipment failures, concerns, needs to the charge nurse. Remove any equipment from use to ensure resident sand staff safety.
  • Carefully use linen, equipment and resident supplies, to avoid misappropriation and waste.
  • Perform skin checks daily while providing resident care and reports any changes to the charge nurse as well as document any skin conditions or changes in the AHT (LTC) Smart Charting system.
  • Utilize pressure-relieving devices as needed per the resident care plan.
  • Review the resident care plan to ensure all of the care needs of the resident are met.
  • Apply splints, orthotic devices as ordered and checks for skin integrity breaks and report any changes to the charge nurse.
  • Utilize gait belts for all residents that require assist for transfers, and or ambulation to ensure resident safety as well as their own safety.
  • Take vital signs and weights per the facility protocol or MD orders under the direction of the charge nurse.
  • Complete daily documentation in the care tracker system indicating the resident behavior, activities of daily living, skin condition, alarms utilization, and any other aspects of resident care per facility protocol.
  • Assist with the admission and discharge process as well as assists transport of residents and belongings associated with transfers to other pavilions.
  • Assist with or provide activities for residents consistent with their needs, wants desires and abilities under the direction of the charge nurse.
  • Follow the facility's Best Practices of two persons using the lifts for any resident transport, transfer, or positioning.
  • Maintain a calm and quiet atmosphere on the pavilions to minimize confusion and frustration as well as to help ensure a positive quality of life for the residents.
  • Be aware that the Charge Nurse, Director of Nursing or Executive Director may make modifications in assignments as needed to meet the physical, spiritual, emotional, psychosocial needs of the resident.
  • Attend in-services relating to dementia, person centered care as well as in services to maintain state and federal requirements for continued certification.
  • Demonstrate initiative and reliability in performing assigned duties.
  • Have the ability to understand and utilize standard precautions and follow established procedures for infection control, hazardous material and all other safety rules.
