The hospice aide is a paraprofessional member of the interdisciplinary group who works under the supervision of a registered nurse and performs various services for a patient as necessary to meet the patient's personal needs and to promote comfort. The hospice aide is responsible for observing the patient, reporting these observations and documenting observations and care performed.

  • Performing personal care needs after given a patient's assignment and attending to his/her requests promptly. If unable to perform a certain task, report to the Case Manager immediately.
  • Schedules patient for visits per the patient plan of care and at the direction of the supervisor; families should be notified at least 4 hours prior to their visit
  • Providing patient and family with positive communication techniques, within realm of ethical and respectful care, including confidentiality.
  • Meeting safety needs of patient and using equipment safely and properly (foot stools, side rails, O2 etc.)
  • Giving personal care including baths, back rubs, oral hygiene, shampoos and changing bed linen as often as assigned.
  • Assisting in dressing and undressing; Planning and preparing nutritious meals, including shopping; feeding the patient as assigned.
  • Providing proper care and observation of patient's skin to prevent breakdown of tissue over bony prominence. Taking and recording oral, rectal and auxiliary temperatures, pulse, respiration and blood pressure when ordered with appropriate completed/demonstrated skills competency.
  • Assessing and reporting on patient's condition and significant changes to the Case Manager. Also aware of the caregiver or other individuals living with the patient and interpersonal issues.
  • Prepares clinical notes, which includes tasks that were performed and reasons assigned tasks were not performed, if applicable
  • Cares for physical, emotional and developmental needs of patient including the need for respect and privacy
  • Is sensitive to client’s specific needs and communicates in a professional and supportive manner
  • Treats clients with dignity and respect while maintaining confidentiality
  • Complies with the requirements of state licensure and accreditation by following policy and procedure maintaining competencies and knowledge base
  • Completes other assignments and duties as requested and assigned to accomplish the goals and objectives of the Company

  • Creative thinker who embraces thought leadership
  • High school graduate is strongly preferred or GED is acceptable along with successful completion of an approved hospice/home health aide training program as required by state and/or federal regulation. May function as a hospice/home health aide if a graduate of a practical or professional school of nursing or has successfully completed a course in a practical or professional school of nursing.
  • CNA license is strongly preferred
  • Current CPR certification
  • Valid driver’s license and automobile insurance

Pay: $18.00 - $21.00 per hour
