Job description
We have partnered with a top hospital in Western NH on their search for a Clinical Coordinator in their Med/Surg Unit. In this role, you will assist the Nurse Manager with the daily activities of the unit and the clinical management of patients. Responsibilities include:
- Coordinate patient care plans by liaising with various providers and medical personnel.
- Admit and discharge patients.
- Coordinate follow-up appointments and refer patients to specialists as needed.
- Collect and input patient data, insurance information, and financial information into the computer system.
- Meet with patients and families to ensure needs are being met and resolve issues as they arise.
Qualifications:
- Active/clean RN license
- 5+ years of clinical experience
- 3+ years of leadership experience
- Excellent communication, organizational, and time management skills
Additional info:
- Full-time, permanent position
- Compensation $72,000-$94,000 depending on experience
