JOB DESCRIPTION

We are currently hiring for a Clinical Coordinator and looking for someone that has a desire to empower our staff to serve others with integrity and compassion, and to be part of an award winning, “Great Employers” recipient, skilled nursing community. The Clinical Coordinator's role is to plan, organize, develop and direct the overall operation of our Nursing Service Department and to ensure that the highest degree of quality care is maintained at all times.


PRIMARY RESPONSIBILITIES

  • The Clinical Coordinator assists the DON to organize, and direct the home care department within the facility, in accordance with current rules, regulations, and guidelines that govern home care.
  • Develop, maintain, and periodically update the Nursing Procedures Manual, written policies and procedures that govern the day-to-day functions of the home care department.
  • The Clinical Coordinator will develop methods for coordination of services with other entities to ensure the continuity of the residents’ total regimen of care.
  • Complete appropriate assessments of potential residents to ensure safe and appropriate care from home care agency.
  • Provide the DON with information relative to the care needs of the resident and the home care department’s ability to meet those needs.
  • Participate in the development of timely in-service training classes and client specific orientation to ensure a well-educated care staff.
  • Develop a written plan of care for each resident that identifies the needs and care of the resident, setting goals, and which professional service is responsible for each element of care.

Qualifications
  • The Clinical Coordinator will have, as a minimum, a Nursing Degree from an accredited college.
  • Will have, as a minimum, three (3) years of experience as a supervisor in a hospital, nursing care facility, or other related health care facility.
  • Will have a current, unencumbered, active license to practice as a Registered Nurse in this state.
  • Will demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served.
  • Demonstrated ability to read, write, speak and understand the English language to communicate.
