Lighthouse Behavioral Health Solutions (Lighthouse) provides individual, outpatient, intensive outpatient, partial hospitalization, residential, psychiatric services, and medication assisted treatment for mental health and substance abuse treatment needs.

Job Summary– This position provides case management services to adults with identified substance abuse and/or

Duties include, but are not limited to:

  • Handle case assignments, draft service plans, review case progress and determine case closure.
  • Routine case management duties as part of the continuum of services to meet each client’s needs.
  • Follow-up on referrals to other community providers to ensure continuity of care and efficiency of services across systems
  • Provides linkage and assistance in accessing inpatient SUD treatment, as needed
  • Transports clients, as needed
  • Completes clinical documentation in a timely and accurate manner.
  • Encouraging, supporting, and listening to clients
  • Maintaining good working relationships with support networks, government resources, and community resources

Case Manager Requirements:

  • Possesses superb written and spoken communication skills
  • Excellent interpersonal skills with clients, colleagues, and community resources
  • Excellent time management skills; organized and able to prioritize
  • Comfortable in a fast-paced environment with multiple clients
  • Able to organize and manage large amounts of files, schedules, dates, and information
  • Calm and cool-headed with a positive attitude
  • Self-directed and self-motivated
  • Comfortable with building personal relationships and dedicated to helping others

Qualifications and Skills:

Required:

  • CDCA
  • Bachelor’s Degree in a behavioral health field (Social Work, Psychology, Counseling, etc.)
  • State of Ohio Driver's License

Preferred:

  • LSW
  • Master’s Degree in a behavioral health field (Social Work, Psychology, Counseling, etc.)

Schedule

Monday - Friday 8:30am - 5pm
