Decatur Wellness and Rehabilitation Center, a skilled nursing facility located in DECATUR TN is seeking a qualified CNA. SHIFTS: 6:30am - 6:30pm / 6:30pm - 6:30am

Must be a certified CNA in the state of TN.

CNA Job Responsibilities & Duties:

• Provide supportive services to nurse(s) and other staff as needed & performs duties as assigned.
• Assist residents with activities of daily living in accordance with care plans, established policies and procedures.
• Assist residents with lifting, turning, moving, positioning, and transporting into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc.
• Coordinate dining room services at assigned mealtimes, including set-up and clean-up, meal tray delivery, feeding assistance, and documentation of meal intake.
• Deliver and document nutritional supplements to residents at assigned times and provides assistance as necessary to ensure intake.
• Assist with weighing residents according to facility policy and record weight.
• Assist with tracking the condition of the resident’s skin.
• Keep nurse and supervisor informed of factors that interfere with being able to perform the work as assigned. Report changes in a resident’s condition to nurse and supervisor.
• Maintain confidentiality of protected health information, including verbal, written and electronic communications.
