What We Are Looking For

Lorian is currently hiring for a Per Diem Certified Home Health Aide (CHHA) to provide optimum care to our home bound patients in

North Orange County

Required Education and Experience*

*
Must meet educational/experience requirements outlined in the laws of licensure states.

  • Certified/licensed and in good standing within state currently working in according to clinical type.
  • The CHHA must complete at least 12 hours of in-service training during each 12 month period. This in-service may occur while an aide is furnishing care to a patient.

Additional Qualifications*

Possesses and maintains current CPR certification (BLS required).

  • Possesses and maintains a valid driver’s license.
  • Possesses and maintains an automobile that is insured in accordance with LORIAN requirements.
  • Ability to read and follow written instructions and document care given in the patients’ electronic medical record.
  • Possesses excellent observation, good nursing judgment, and good oral and written communication skills.
  • Is self-directing and has the ability to work with little direct supervision.
  • Is flexible and cooperative in fulfilling the role obligations.
  • Ability to work with adults (18 years and older), demonstrating tact and patience.

Physical Requirements*

Is able to present a pre-employment physical which states they are free from health conditions which would interfere with the employee’s ability to perform assigned duties, is free from signs or symptoms of infectious disease and is signed by the person who performed the assessment. A similar physical is required annually thereafter.

  • Is able to pass a pre-employment Protein Purified Derivative (PPD) tuberculin skin test and annually thereafter. If unable to pass PPD testing or have history of positive PPD testing, a Chest X-ray will be required of a new employee and every five (5) year thereafter.
  • Maybe subjected to prolonged standing or sitting.
  • Able to lift, position, or transfer patients as needed.
  • Considerable amount of reaching, stooping, bending, kneeling or crouching.

Job Limitations*

The HHA will not function in any manner viewed as the practice of nursing according to the State’s Nursing Practice Act. Specifically, the HHA will not administer medications, take physician’s orders or perform procedures requiring the training, knowledge and skill of a nurse, specifically sterile techniques.

Note: Documentation must be accurate, complete, and submitted to LORIAN before visit is deemed acceptable and payable to CHHA

Job Summary*

The Certified Home Health Aide (CHHA) is a paraprofessional member of the care team who works under the supervision of a RN/therapist and performs various personal care services in a safe and efficient manner as necessary to meet the specific patients/client’s needs via written instructions. The CHHA is responsible for observing patients/clients, reporting these observations, and documenting observations and care performed. The CHHA will be assigned in a manner which promotes quality, continuity, and safety of patient/client care.

Responsibilities

*
A CHHA provide services that are:

  • Ordered by the physician;
  • Included in the plan of care (POC);
  • Permitted to be performed under state law; and
  • Consistent with Home Health Aide training.
  • Responsibilities of the CHHA include, but are not limited to, the following:
  • Personal Hygiene and grooming including
  • Bed Bath;
  • Sponge, tub, and shower bath
  • Hair shampooing in sink, tub, and bed;
  • Nail and skin care;
  • Oral hygiene;
  • Toileting and elimination;
  • Safe transfer techniques and ambulation;
  • Dressing and undressing patients/clients
  • Skin care and prevention of skin breakdown
  • Keeping patients/clients living area clean and orderly, as appropriate
  • Assists with planning and preparing nutritional meals.
  • Assists in feeding the patient/client, if necessary.
  • Takes and recording oral, rectal, and axillary temperatures, pulse, respiration, and blood pressure when ordered (within the training limitations of the aide).
  • Assists in ambulation and exercise according to the POC.
  • Performs range of motion and other simple procedures as an extensional therapy service as ordered (within the training limitations of the aide).
  • Assists patient/client in the self-administration of medication (only).
  • Washes patients/clients laundry, as appropriate.
  • Meets safety needs of patients/clients and using equipment safely and properly (foot stools, side rails, restraints, etc.)
  • Maintains an open communication process with the patient, caregiver or family members.
  • Demonstrates competency with assigned tasks.
  • Complies with infection prevention and control policies and procedures.
  • Honors patient rights.
  • Reports on patients/clients condition and significant changes to the registered nurse or other appropriate skilled professional, and must complete appropriate records in compliance with LORIAN’s policies and procedures.
  • Adheres to LORIAN’S documentation and care procedures and standards of personal and professional conducts.
  • Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attending and participation in continuing education, in-services and clinical meetings.
