Primary Health Care (PHC) was founded in 1981 by Dr. Bery Engebretsen in Des Moines, IA. Our mission has remained unchanged since that time, to provide healthcare and supportive services to all, regardless of insurance, immigration status, or ability to pay.

Based on the needs of the communities we serve, PHC offers a spectrum of medical and dental services including family practice, behavioral health, HIV care and services, and pharmacy. PHC’s Homeless Support Services is the entry point for serving people experiencing homelessness in Polk County. Enabling services are available to help patients with benefits enrollment, case management, transportation, translation, and patient education. We currently have locations in Ames, Des Moines, & Marshalltown.


As a Centralized Intake Worker you will be responsible for completing Centralized Intake interviews, assessments, and placement and referral activities in the PHC Homeless Support Services Department or in the community to assist with housing placement, case management, and locate income streams in order to prevent or respond to homelessness. You will provide quality customer service, ensures accurate client data, and coordinates with team members to provide referrals to community agencies, emergency shelters, and housing programs.

What You Will Be Doing

  • Provides excellent customer service by assisting clients to ensure their housing related needs are met.
  • Completes accurate, live data entry into HMIS according to Centralized Intake (CI) policies and procedures.
  • Provides referrals to clients based on client eligibility for programs and/or mainstream resources.
  • Remains compliant with all policies and procedures, including CI policies, PHC policies, and all grantor policies or requirements.
  • Answers and responds to client contact, including phone calls or in-person interactions, in a timely manner and records accurate and complete notes in HMIS.
  • Maintains confidentiality, except in situations where there is a release of information signed by the client, or in situations of actual or threatened harm by or to the client.


Qualifications You Need to Bring

Education: Bachelor’s degree. Four years’ experience in essential functions of position may be substituted for Bachelor’s degree if allowed by granting agency.

License/Certifications:

  • Valid Iowa Chauffeur’s Driver’s License
  • Current Mandatory Reporter Training: Child and Dependent Adult Abuse recognized by the Iowa Department of Public Health.
  • Must be obtained during introductory period if not current.

Your experience and skills determine your base pay. The hiring range for this position is typically $17.00 – 25.50. PHC also offers a comprehensive benefits package, including:
