This opportunity is for a Supervisor to manage the Somerset County Supports Coordination Organization (SCO) unit. Assure quality services, provide advocacy, review Individual Support Plans, provide supervision to Supports Coordinators, review documentation, and oversee all departmental functions.

DESCRIPTION OF WORK

This position serves as the Supervisor of the Somerset County Supports Coordination Organization (SCO) and is responsible for coordination of social services to approximately 350-400 individuals in assuring quality and continuity of care through a comprehensive supports coordination and consumer advocacy system. Prepare for and conduct weekly departmental meetings. Review all submitted Individual Support Plans (ISP) for accuracy prior to forwarding to the Administrative Entity (AE). Provides supervision and management to Somerset Supports Coordinators (SC) (County Caseworkers) on a daily basis. Assist staff with the completion of all documentation and reviews for thoroughness. Provide support and technical assistance with HCSIS and EIM. Responsible for supporting all aspects of Incident Management. Handle routine program issues and report these concerns when necessary to the Supervisor. Attend supervisor’s meetings and share information with SC’s and clerical staff as needed. Coordinate the dissemination and review of information from Office of Developmental Programs (ODP), for example bulletins, informational communications, waiver requirements, etc. to all concerned individuals, including SC’s, providers, participants, and families. Screen and review all new referrals received from the AE/County DS Program Intake Department and assign to a Supports Coordinator. Conducts file reviews. Develop, through individual and group conferences, the supports coordinators efficiency and skill in determining the provision of services to ensure that consumers and/or families are receiving and utilizing all available resources. Communicate productivity standards to staff and monitor a variety of reports. Maintain a small caseload to include development of ISP’s and individual monitorings. Assist with projecting program/private provider needs, making recommendations for expansion of programming or changes to existing programming, to provide quality consumer care. Recruit, interview, select and employ all required personnel within the SCO. Orient new SC’s and assist them in implementing and meeting their responsibilities to consumers, workers, agency staff, and the community. Complete employee performance reviews. Attend in-service training. Perform other related duties as required.

Work Schedule and Additional Information:


  • Full-time employment.
  • Work hours are 8:00 am to 4:00 pm, Monday - Friday, with 30-minute lunch.
    • On occasion, staff may need to work hours outside of the set work week to complete projects, meet work demands, or in the event of an emergency.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

QUALIFICATIONS
Minimum Experience and Training Requirements:

  • Two years of professional experience in public or private social work and a bachelor’s degree with major coursework in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences; or
  • Any equivalent combination of experience and education which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences.
