DEPARTMENT DESCRIPTION:
CHA PACE is a Program of All-Inclusive Care for the Elderly (PACE) at Cambridge Health Alliance (CHA). CHA PACE serves adults 55 years and older with chronic medical and/or mental health difficulties with the goals of achieving the highest level of community independence and preventing nursing home placement. CHA PACE's high functioning interdisciplinary team works to fully integrate and coordinate medical and behavioral health care, community resources, and in-home support services.

Job Duties:
1. Actively frames interventions based on the PACE model of care in collaboration with the IDT and participant’s plan of care.
2. Develops, coordinates and implements discharge plans consistent with participant’s goals and IDT recommendations.
3. Acts as participant navigator for new enrollees and their families.
4. Acts as a resource to the MSW staff regarding participant’s concrete service needs.
5. Provides support to participants through empathic, trusting relationships.
6. Documents in the medical record.
7. Other duties as assigned

MINIMUM QUALIFICATIONS: Education/Training:
BS in related health field or relevant experience

Work Experience:
Experience working with relevant community groups preferred. Demonstrated experience working in multicultural and multidisciplinary settings required.
One year experience working with the geriatric population.
At least 2 years experience with Care Coordination, Discharge Planning, Utilization Review, or similar.

Other information:
Bicultural and/or bilingual in the specific cultural/language group of the population/community served in a particular job.
Reliable transportation for home visits.
Flexibility, initiative, creativity, and excellent organizational skills.
