JOB SUMMARY

This position will serve as Case Manager for AOT (Laura’s Law) services and subsequent Assertive Community Treatment (ACT) eligible adults with serious mental illness in Contra Costa County. The Case Manager provides case management services to clients who are difficult to engage and may deny the need for mental health services. The Case Manager coordinates care for clients and assists them in accessing services and resources that will help maintain them in the community in the least restrictive environment. In addition, the Case Manager acts as a liaison between treatment providers and the client and assists clients with the development of social and community support systems that promote stability and enable clients to function at the highest level possible. The Case Manager offers extensive knowledge of community resources and support as a primary means of assisting each client to reach their rehabilitation and recovery goals. The range of authority is consistent with that of MHS Case Managers.


RESPONSIBILITIES

  • Provides service coordination / case management for a shared group of clients including coordinating and monitoring activities of the treatment team. Cooperates with Program Supervisor and Program Manager in rotating client visits/assignments with the treatment team. Successfully functions as a team member.
  • The majority of ACT services are provided in the community and client residences and requires client transportation in personal and/or company vehicles. Provides on-call, after-hours, and weekend/holiday services as required by contract.
  • Develops, evaluates, and implements treatment goals and plans in conjunction with the treatment team.
  • Provides individual supportive coaching in all areas of clients’ life, as well as education of symptom management in

consultation with county liaisons.

  • Performs psychosocial assessments. Performs Risk Assessments. Maintains a daily log of client contacts.
  • Coordinates the full range of client’s needs. Provides brokerage, linkage, and advocacy to ensure resources and services

are accessed in a timely manner, providing community-based services in client’s residence as needed.

  • Conducts and oversees psychoeducational groups as directed by Program Supervisor and/or Program Manager.
  • Performs shift management duties in coordination with other ACT staff as directed by Program Supervisor and/or

Program Manager. Functions as assistant to shift manager when assigned. Follows administrative chain of command.

  • Complies with all charting and documentation requirements per contract, funding agency, and MHS policy.
  • Prepares critical care issues for discussion during supervision and Daily Organizational/Morning Meetings.
  • Provides information about the program to community agencies connected to the behavioral health services system.
  • Completes all assigned duties within the time frame and according to the standards as established by the Program

Manager.

  • Assertive Community Treatment model as it relates to client care.
  • Knowledge of community resources that will be utilized by clients.
  • Cultural and socio-economic factors that inform service provision to persons with psychiatric or dual diagnosis disorders.
  • Procedure manuals and an ability to interpret these effectively to staff, family members and community providers.
  • Interact positively with persons of diverse socio-economic backgrounds and education.
  • Foster excellent rapport with clients and build trusting and motivating relationships.
  • Comply with all MHS and program rules, policies and procedures including HIPAA standards.


QUALIFICATIONS

  • Master’s degree in a behavioral science and at least two years’ experience providing clinical services to individuals with mental illness.
  • Licensed preferred, or license eligible required with Board registration (ASW, IMF, License Psychologist).
  • Knowledge and experience with Microsoft Office Suite and EHR software
  • Excellent organizational and time management skills
  • Strong verbal and written communication skills.
  • Valid California driver’s license and current auto insurance per contract requirements
