Summary
The Case Management will be responsible for providing structured reintegration services to safely transition individuals adjudicated Not Guilty by Reason of Insanity (NGRI) from the State Operated Psychiatric Hospital (SOPH) to the community with the goal of decreasing secure inpatient lengths of
stay, reducing readmission and recidivism. They will provide clinically appropriate Medicaid billable services based on an individualized treatment plan, liaison with the SOPH to ensure consumer transition to the community, prepare and submit statutory reports to the criminal court, provide access to Individual Placement Support (IPS) where possible, develop plan for progressive privileges & monitor compliance with the conditional release plan in collaboration with the Forensic Community Administrator and streamline re-hospitalization when necessary. This position also ensure that all services rendered meets rules and regulations of the Illinois DHS Division of Mental Health via Rule 132.
Role and Responsibilities

  • Submit performance and financial reports no later than 30 days after the quarter ends.
  • Submit a Close-out Reports no later than 60 days after this Agreement's end of the period of performance or termination.
  • Ensure that periodic reports are generated and are submitted to the criminal courts in a timely manner, pursuant to the requirements of Illinois statutes 730 ILCS 5/5-2-4(a-1)(D);
  • Provide and ensure prompt access to clinically appropriate mental health and substance abuse services identified in the Conditional Release plan
  • Participate in regular training and collaboration with DHS/DMH Forensic Services related to forensic reporting and progressive privileging
  • Liaison with DHS/DMH to make timely reports of staff changes involved in care of Conditionally Released consumers
  • Track the number of conditionally released in the supervised residential program.
  • Track the number of NEW consumers (during reporting period).
  • Track the number of consumers with a written plan that addresses all conditions of release within 30 days of entry into program.
  • Track the number of critical events.
  • Track the number of NGRI consumers in supervised residential programs who received all mental health, substance abuse, and employment services, as necessary, as indicated in the Order of Conditional Release.
  • Track the number of consumers discharged or released to another court approved level of care.
  • Track the number of consumer progress reports submitted to the courts and DMH within 90 days of release and every 90 days thereafter or as ordered by the court.
  • Track the number of special progress reports requested and submitted by DMH or a Judge.
  • Visit state and community hospitals, and the larger community to link with consumers in need of the program services.
  • Complete partial intake on consumers at community agencies to determine needed services;
  • Link with the consumer at Intake and provide case management services of fiscal and housing resources and medication monitoring and/or other health issues within a 30-day period. Assessment of the consumer’s current and potential strengths, abilities, preferences and needs will be provided.
  • Work with family and significant other of the consumer when possible to restructure maladaptive family relationships through family conferences, mental health education, crisis intervention and advocacy;
  • Interface with community services, treatment programs and natural support systems on behalf of the consumer to ensure that the consumer’s needs are met efficiently and effectively;
  • Monitor the quality of all services provided to the consumer to ensure that all necessary services and resources are received;
  • Complete all documentation, correspondence and data entry in a timely manner. In accordance with Agency`s standard and procedures;
  • Reassess the consumer’s progress in treatment.
  • Attend weekly supervisory session to discuss caseload activities and monitor performance;
  • Ensure all discharge paperwork is received at point of entry to BEW
  • Complete daily documentation In Carelogic computer system
  • Apply for SSI and link card benefits, Apply for Identification at the Secretary of State
  • Register consumer for Genoa pharmacy upon Admission
  • Correspond with states attorney via phone and email
  • Attend ALL court appearances (In person or Via zoom due to the pandemic)
  • Coordinate with substance abuse or Residential or IPS Case managers as needed.

Qualifications

  • Bachelor’s degree from an accredited college or university in Psychology, Social Work or other social service related field, or LPN or RN License with a minimum of two (2) years of Psychiatric Experience;
  • Valid Illinois driver’s license.
  • Reliable transportation and be willing to transport members to appointments and for other services.
  • Possess strong written and verbal communication skills;
  • Must be Computer Literate. Experienced with Microsoft Office;
  • Experienced with Electronic Health Record (EHR) preferably Carelogic computer system.
  • One year experience working with persons who have severe or persistence mental illness is desired but not required;

Service oriented, empathetic and able to interact and develop rapport with a diversified population;
