Location: 600 N Wolfe Street, Baltimore MD 20287

Shift:

Full Time (40 hours; 4 10 hr. shifts)

8:00AM - 6:00PM

Exempt; Pay Grade: 1C

Job Summary:

In collaboration with patients/families, social workers, physicians and the interdisciplinary team, the Case Manager provides leadership and advocacy in the coordination of patient-centered care across the continuum to facilitate optimal transitions and progression in care. The Case Manager manages clinical resources and transition planning for patients within an assigned caseload from pre-admission through post-discharge, actively working to identify/ eliminate barriers to the delivery of clinical services with the patient, family and care team.

Education:

BSN required. Must be from an accredited nursing program.

Licensure/Certifications:

  • Active Maryland or Compact RN license is required
  • BLS CPR certification required: AHA, Red Cross, or Military Training Network accepted

Experience:

2 years of acute care RN experience in M/S, oncology preferred or other subspecialty training for a minimum of 2 years.

Knowledge/Skills:

Ability to role model and support practices consistent with evidenced based care, guide others with plan of care, independently make clinical decisions in complex situations, and proactively assist others with patient assignment.
