House of Possibilities (HOPe) and Jewish Vocational Service (JVS) have partnered to launch Career & Community Access Program (CCAP), a new program to provide career and community access for individuals with disabilities living in the Boston area. The Case Manager role is crucial in developing independence, building confidence, and supporting our Members towards living a full and meaningful life. The HOPe Case Manager combines a smaller caseload with direct care, supporting adults 18+ with a variety of disabilities in our Career & Community Access Program. In this role you will:

• Manage and implement Members’ individual service plans based on needs and goals, and record daily progress, for Community Based Day Support and Employment Support services
• Promote community involvement and safety
• Facilitate activities related to members' pathways to employment and inclusion in their community
• Teach Members how to manage activities of daily living such as cleaning, cooking, shopping, budgeting, personal hygiene, health care practices, money management, traveling, self-control and leisure time
• Identify areas of crisis and defuse the situation, ensuring the safety of all concerned
• Administer medication according to MAP regulations, as applicable
• Assist members physically as needed
• Transport Members to community activities or employment opportunities as necessary
• Will pay premium for bilingual candidates - Spanish preferred.

Job Type: Full-time

Pay: $18.00 - $25.00 per hour
