JOB SUMMARY: Under the supervision of the Clinical Supervisor, the Case Manager/ Care Coordinator will provide case management and coordinate services for clients and outside individuals seeking support, including oversight and electronic documentation of all such services.

ESSENTIAL DUTIES

  • Coordinate and provide direct case management for clients to services such as healthcare, employment, housing, transportation, and community resources, and aiding with applications/ documentation as needed.
  • Analyze and evaluate individual situations, make independent, sound decisions, and take appropriate action.
  • Develop and maintain database of community resources.
  • Coordinate referrals to and from outside agencies.
  • Collect and electronically document all care coordination services in required databases, as per contracts.
  • Develop communication/ documentation system for all care coordination services provided by the agency to be recorded electronically.
  • Work closely with intake and discharge teams to provide case management and referral information.
  • When the driver is not available, may assist with transportation.

SECONDARY DUTIES

Perform other tasks as assigned by supervisor, as needed in emergencies, or supporting other staff with supervisor approval.

JOB REQUIREMENTS:

This position requires the employee to be available during afternoon and early evening hours, while the majority of clients are on site. The Client Care Coordinator should possess a strong knowledge of resources in Santa Cruz and the surrounding community.

EDUCATION REQUIRED:

  • Associates degree or higher in related field.

LICENSES/CERTIFICATIONS REQUIRED:

  • Must become registered recovery worker with accredited credentialing board, CCAPP or CAADEE – may be obtained after start of employment.
  • California Class C driver license.
  • The following are required for STOP clearance and may be obtained after start of employment:
  • Live Scan security background check, current CPR/First Aid certification, TB test, Health Screening Clearance.

EXPERIENCE REQUIRED:

  • Two years' experience in social work or case management preferred.
  • One year of previous experience in assisting clients to access services in Santa Cruz County.
  • Strong computer skills required.

KNOWLEDGE REQUIRED:

  • Principles of casework, social work, and case management.
  • Healthcare, education, housing, and employment requirements and labor market conditions.
  • Computer systems and applications, including county EHR Avatar.
  • Community service providers and resources.
  • Principles of eligibility determination.

QUALIFICATIONS:

Bilingual (Spanish/English) preferred; Market knowledge.

  • Ability to present information or ideas clearly in English, both orally and in writing. This includes ability to proofread and use correct spelling, grammar and punctuation, and ability to write correspondence and memos.
  • Ability to understand and follow instructions, reason and make judgments as they relate to the job or situation.
  • Ability to interface effectively with all levels of employees, the public and with clients; to diffuse hostility when needed.
  • Minimal proficiency and/or willingness to learn computer skills is necessary. This includes Microsoft Office, Office 365, Google Apps, and other Web applications.
  • Ability to problem solve and resolve conflicts especially in high pressure situation.
  • Ability to adapt to situations and understand other's feelings and personal viewpoints.
  • Ability to work with and relate to other professionals.
  • Ability to work and function as a team member.

JOB SETTING

The job is performed in an enclosed climatically controlled office. Work is completed independently and in cooperation with the Clinical Supervisor, Program Manager, other company personnel, the public, and clients.

PHYSICAL DEMANDS

The job is primarily sedentary with walking and standing as is necessary to carry out the job duties. Normal lifting and/or carrying is limited to approximately 15 lbs., with occasional lifting up to 25 lbs. Heavier items are moved by using proper material handling equipment, assistance from another person or by work order request.

Normal or corrected talking, hearing and seeing abilities are sufficient to perform the required tasks.

Pay: $25.00 - $29.00 per hour
