- Performs case management services for women with Perinatal Hepatitis B utilizing appropriate procedures.
- Obtains health histories for women with Hepatitis B.
- Reviews communicable disease reports to ascertain pregnancy status and the need for case management, infant prophylaxis, and education.
- Provides information to clients and families regarding Hepatitis B and prevention of the spread of the disease.
- Provides education to health care providers and hospitals regarding perinatal Hepatitis B.
- Refers clients to medical home.
- Works with other staff trained in perinatal Hepatitis B case management to ensure a prompt initiation of case management.
- Monitors and revises procedures with the approval of the Medical Director.
- Compiles reports regarding Perinatal Hepatitis B for department of state health services.
- Serves on departmental committees.
- Performs all other related duties involved in the operation of the Public Health Department as assigned or required

TO APPLY, must have a Bachelor's degree or higher in Nursing (BSN) or Social Work (BSW).
TO APPLY, must have Two (2) full-time years of related work experience.

Good knowledge of fundamental concepts, procedures, and case management.
Good verbal and written communication and presentation skills.
Ability to make professional case management decisions and operate autonomously under procedures.

Tarrant County will conduct background checks on new hires that will include a criminal background check related to convictions and deferred adjudications in the past seven years and may include credit reports, motor vehicle records, employment records and educational attainment.
