Health Services Care Manager

This is a full-time (35 hour/week) Local 509 Union position with a hybrid remote schedule (2 days/week) and the option of a flexible 4-day work week.

In this role you will be responsible for assessing, reassessing and monitoring consumers for need and eligibility for in-home services, recommending/approving service plans, providing information on available resources, and coordinating in-home/community services.


Compensation:
Starting annual salary is $46,347.94.

What you'll be Responsible for:

Essential functions of the position are below. Additional duties may be assigned as required.

  • Conduct initial and on-going assessment of consumers' health and functional status.
  • Conduct visits as required in various settings including consumers' homes, community settings, and/or medical facilities.
  • Participate in development of consumers' care plans, including determining appropriateness and need for in-home/community-based services.
  • Participate as part of consumer's primary care team including, attending team meetings, trainings, and joint home visits.
  • Arrange, coordinate, and oversee in-home and community-based services for consumers.
  • Monitor consumer's health status, in conjunction with care team, to assess for effectiveness of current services, need for additional services, and/or need for long-term placement.
  • Assist consumers with MassHealth redeterminations and other benefits paperwork, as requested and appropriate.
  • Maintain consumer records in accordance with standards.
  • Compile and submit required reports.


Qualifications:

  • A Bachelor's degree in Social Work or a related field.
  • Social Work license preferred.
  • Knowledge of and experience working with older adults in a community-based setting preferred.
  • Strong interviewing and assessment skills.
  • Strong interpersonal and organizational skills.
  • Excellent written and verbal communication skills.
  • Computer skills: Proficient with MS Office Suite and entering narrative and other data into a database. Ability to use multiple databases. Ability to use the Internet to conduct information searches
  • Private transportation.
