Job Description

We are seeking a full-time, skilled, dedicated Case Manager specializing in services for people with disabilities to work within a multi-agency South Brooklyn initiative that focuses on helping families having adult children with Autism and/or I/DD plan for their futures. Working with three agency partners, the HES, the Shorefront YM&YWHA and Kings Bay YM&YWHA, the Case Manager will provide culturally competent, strengths-based services that engage adults with ASD/DD and their families to facilitate their future planning process through individual and group discussions. The Case Manager will also link families to necessary resources and plan educational seminars presented by legal, financial and life planning experts. Knowledge of the OPWDD system is essential. The Case Manager should be able to work as a passionate member of collaborative teams, work within complex environments and utilize excellent judgment. Case Management services will largely be conducted through remote service delivery, including video-conferencing although there will be some on-site service delivery at each participating organization, respecting public health considerations.

Job Responsibilities

The Case Manager for Adults with Disabilities & Their Families will work in collaboration with the Directors of Programs for People with Disabilities at The Hebrew Educational Society, The Shorefront Y and the Kings Bay Y. The Case Manager will report directly to the HES Associate Executive Director in carrying out the following job responsibilities:

  • Using a variety of outreach strategies including phone, email, social media, fliers and direct mail, identify adults ages 30-50 with Autism or I/DD and their family members that reside in South Brooklyn.
  • Engage adults with ASD/DD ages 30-50 in consideration of their futures through group and individual discussions.
  • Engage families having an adult family member with ASD/DD, ages 30-50, in planning discussions to include legal and financial considerations, housing, care planning and identification of critical issues. The Case Manager will help families develop their respective support systems to assist the adult having ASD/DD throughout their lives.
  • Maintain ongoing documentation of all case management activities.
  • Develop and monitor case plans with each family and adult with ASD/DD engaged in the program.
  • Coordinate occasional evening and weekend family engagement and educational programs, as well as outreach to community partners.
  • With agency partners, plan and implement several community events such as a Future Planning Fair at two of the partner sites.
  • Participate in ongoing program evaluation and program development with agency partners and recipients of services.
  • Work with managerial, administrative and direct service staff at each agency to promote program goals and maintain high-quality standards that includes involvement with agency-wide activities and interdepartmental projects.
  • Participate with agency partners in reporting and presenting to the funder – UJA-Federation of NY
  • Take on other assignments as per the program’s development and changing needs identified by the Hebrew Educational Society, the partners and UJA-Federation of NY.
  • Qualifications:
  • A relevant Bachelors’ Degree, MS or MSW preferred
  • Ability to travel to three agencies in South Brooklyn with locations in Canarsie, Sheepshead Bay and Brighton Beach
  • Ability to speak Russian a plus
  • Some evening and Sunday hours will be required on occasion
  • Proficient in Microsoft Office and G-Suite
  • Approximate schedule: M-F 10am-6pm + some occasional weekends
