Position Summary:
The Unaccompanied Children (UC) Transitional Foster Care (TFC) Medical Coordinator is primarily responsible for maintaining accurate and timely medical records, appointments, and services in accordance with the requirements of the program. This position is expected to function effectively with regular supervision while following the guidelines given on procedures, along with agency, federal, and state regulatory requirements.

The Medical Coordinator partners with other health professionals regarding health and safety standards for out-of-home child care, child care licensing requirements, disease reporting requirements for care providers, immunizations of children, medication management. This position maintains knowledge of community health and mental health resources for children.

$2,000.00 signing bonus with 1/2 paid of bonus paid on 1st full 2 week worked paycheck following hire and remainder of the signing bonus on the first full 2 week worked paycheck after 9-month work anniversary.

Wage Range: $14.84-$18.33 hourly

Essentials Duties and Responsibilities:
  • Schedules, coordinates, and follows through on the medical and dental services provided to clients to assist case managers.
  • Sets up and maintains the medical record for each program client.
  • Ensures the initial medical screening, TB screening, immunization records, and medical recommendations are included in the medical record.
  • Conducts frequent concurrent audit reviews to ensure data entry and medical records are accurate and correct.
  • Addresses specific medical conditions with medical providers and resources, while serving as a liaison with the provider and foster family.
  • Assists foster parents and case managers in addressing any barriers that interfere with timely medical services for clients.
  • Assists the program with communication medical needs, conditions, recommended treatment, and medical emergencies to office of refugee resettlement (ORR).
  • Maintains accurate, up-to-ate information on clients in care while meeting ORR and CCWM's documentation and reporting requirements.
  • Stays informed of all agency, federal, and state regulatory requirements related to social services.
  • Attends trainings and meetings as needed.
  • Drives for Agency business.

  • Ability to demonstrate computer and typing skills sufficient to perform essential job functions.
  • Ability to relate to diverse populations and cultures while communicating with clients and the public in a courteous and effective manner.
  • Knowledge and experience with medical systems and procedures.
Recommended Employment Qualifications:
Education:
A high school diploma or GED is required.

Experience:
  • A minimum of one year experience working in the child welfare field working with children and/or adolescents in a social service setting is required.
  • Bilingual Spanish/English fluency is highly preferred.
Certificates, Licenses, Registrations:
  • None required.
Supervisory Responsibilities:
This position does not have supervisory responsibilities.
