Summary
Provide day to day leadership and direct support with personal care, job coaching, job specific training, employer negotiations, education to employer on best learning techniques, accommodations, and skill development training for individuals supported by Incompass Human Services.
Schedule: Monday-Friday: 8:00am-4:00pm
Essential Job Functions
  • Lead activities and training for individuals that enhance their employment skills and specialized service need areas with the focus on continual improvement and/or maintenance of their abilities in coordination with an individualized career plan.
  • Outreach/marketing to the business community to assist individuals in securing employment and collaboration with community organizations that support job development efforts for the individuals.
  • Coordinate and provide coverage for Employment Worksites and complete job development responsibilities including completion of Employment Portfolio, Job searching, and coaching.
  • Provide personal care and assistance to individuals consistent with both their physical needs and defined support plans.
  • Actively participate in interventions, providing positive, knowledgeable support, and ensuring plans are followed.
  • Provide transportation as requested. Provide on-call coverage and job coaching support during evening and weekend hours as needed.
Education and Experience
  • Bachelor’s degree in a related discipline preferred.
  • At least two years of case management, program management, and behavior plan development experience preferred.
  • Previous experience working with dually diagnosed clients is highly desirable. Candidates with an equivalent combination of education and experiences will be considered.
  • Must have valid driver's license and reliable vehicle to transport individuals.
