POSITION SUMMARY:
Cross Home Care is looking for a Full-Time Care Coordinator to work in our Meadville Office 3 days/ week and our Hermitage 2 days/ week.

This person must be caring and compassionate, able to work in a fast-paced environment, demonstrate problem solving skills, be comfortable communicating with other community professionals, and be willing to travel in (often) Crawford and Mercer counties and (less often) Erie, Venango, Lawrence and Butler Counties.

SHIFTS:

Meadville Office M, W, F 8:00am- 4:30pm; Hermitage Office Tu, Thurs 8:00am- 4:30pm (Subject to change- will discuss in interview)

QUALIFICATIONS:

  • REQUIRED: Must be a minimum of 18 years of age
  • REQUIRED: Driver’s License and your own reliable vehicle
  • PREFERRED: Minimum of 6 months of experience in home care preferred
  • PA/ Federal Background Check- Resulting with Clear Criminal History
  • Good computer skills
  • Excellent written and verbal communication skills.
  • High level of detail
  • Expertise and organizational skills related to file management and compliance.
  • Desire to learn or experience with Scheduling Software
  • Demonstrate self-motivation, self-direction, flexibility and the ability to deal with a high level of stress and constantly changing priorities with enthusiasm.
  • Ability to communicate effectively with clients, families, caregivers, prospective employees and administrative staff from diverse backgrounds.

DUTIES AND RESPONSIBILITIES:

General Office Duties:

  • Answers telephone inquiries and channels them appropriately
  • Address client and caregiver needs using appropriate problem solving skills
  • Performs general office duties including but not limited to word processing, photocopies, files, shreds, sort/distributes mail, provides reception and telephone services.
  • Print, scan, file, email and fax as needed
  • Manage bulk mailing process and office mail functions
  • Participates in all meetings at the office in-person, Zoom, or over the phone.
  • Maintains the files, supplies, and general office condition in an orderly manner.
  • Maintain a clean and inviting office environment
  • Assisting other office staff with all aspects of daily office tasks directed to you as needed.
  • Document daily tasks completed and events that happened each day
  • Understands and adheres to established Cross Home Care policies and procedures.
  • To positively represent Cross Home Care through professional attitude, appearance, work ethics, timeliness and building a strong relationship with each client and employee
  • Other duties as assigned by supervisors

Client Care Coordination Duties:

  • Assist with all client inquiries and complete all client intakes
  • Travel to potential client's homes to complete Client Assessments and Intakes
  • Travel to active client's homes to complete Supervisory Visits and Quality Care Visits
  • Complete Compassion Calls for all active clients and employees
  • Build a strong rapport with clients, Personal Care Aides and other office staff
  • Communicate with all office staff, employees and clients regarding all crucial and important information
  • Work closely with potential and active clients, Personal Care Aides, community professionals and Service Coordinators to establish appropriate care.
  • Discerns client services required as outlined in agreements and care plans.
  • Report all Critical Incidents to appropriate channels and personnel
  • Ensure safety of all clients and direct care workers
  • Use good judgment in decision making situations and always respect confidential matters relating to the client

Marketing Duties:

  • Marketing: Build rapport with professionals in the community and build a referral network
  • Attend marketing functions in the community and represent Cross Home Care in a positive and professional manner

On-Call Phone Duties:

  • Participate in the on-call phone rotation and be available to respond to calls within 15 mins
  • Provide assistance with all communication received through the on-call phone
  • Not be under the influence of any alcohol, drugs or other substances while on-call

Job Type: Full-time

Pay: $13.50 - $16.00 per hour
