Overview:

This role requires that candidates have MHRS (BA + 4 years exp erience ) credentials.

As an Intensive Care Coordinator, you will be vital in facilitating and fostering collaboration among multi-system involved youth, family members, and other support individuals, forming a cohesive Child-Family Team (CFT). By integrating natural and formal system partners, you will effectively address the identified goals and objectives of the child and family, ensuring a unified collective effort to support and bring about lasting positive changes aligned with the Child-and-Family Plan.
Your responsibilities as a Intensive Care Coordinator will include ensuring that CFTs are happening for all ICC clients and creating, conducting, and managing feedback for all CFTMs. With your exceptional communication and organizational skills, you will be able to facilitate collaboration among all parties involved and help build a supportive network around each child and family.

Responsibilities:
  • Understand and apply the principles of the Katie A. Core practice model
  • Establish the client and family team and facilitate discussion to create shared vision for the team in initial meeting and revisit as needed
  • Provide services that are youth and family centered
  • Provide coordination and planning of CFTMs including inviting all team members to CFTMs
  • Create and distribute CFTM Action Plans to team members for every CFTM
Qualifications:
  • Bachelor’s Degree in related social service field
  • MHRS (BA + 4 years experience) credential required
  • Own vehicle, valid California Drivers License and insurance
  • Ability to work as part of a multidisciplinary team
  • Bilingual skills are preferred
  • Strong organizational, writing and communication skills
  • Community based experience is preferred
  • Clearance of TB test, fingerprints, and any other mandatory State/Federal licensing or certification requirements
  • Proof of COVID-19 vaccination and booster
Schedule:
  • Monday to Friday
  • 9:00 am- 5:00 pm
  • Flexible schedule with ability to work evenings and weekends as needed
Benefits:
  • Starting at $31.00- $33.50 per hour, with actual salary dependent on creditable experience above the minimum qualifications for the role
