Description:
The Assessment Worker is responsible for reviewing hospital or clinic records and interviewing families, prenatally or immediately after birth, to determine families eligible for services through the Healthy Families/Healthy Start Program. Conducts comprehensive assessments at St. Joseph's Women's Hospital and in the field using standardized protocols and instruments, and designs individualized interventions for families designed to improve pregnancy and childhood health outcomes. Performs other duties as assigned. Required Education and Experience included Bachelors in Related Field with 2 years experience working with Women, Children or Family Services or High School Diploma with 3 years experience with Healthy Families program.
