Sycamore Springs is a 48 bed behavioral hospital located in Lafayette, IN. The hospital offers inpatient and outpatient mental health and addiction treatment to adults, adolescents and their families. At Sycamore Springs we are dedicated to Changing People’s Lives®.

The Assessment department plays an essential role in helping fulfill our mission of “ Changing People’s Lives®”. Assessment team members assess the needs of walk-in and phone callers and completes initial psychiatric assessment on patients seeking treatment. As an Assessment Coordinator, you will provide support to staff providing comprehensive assessment and quality therapeutic care to patients seeking treatment of substance abuse, dual diagnosis or psychiatric or emotional disorders. Position Details:
  • Coordinator/LPN assess the needs of walk-in and phone callers to ensure their referral to the service or resource to best address those needs.
  • Coordinator/LPN assists in scheduling initial psychiatric assessment on patients seeking treatment of substance abuse, dual diagnosis or psychiatric or emotional disorders and presents symptoms to psychiatrist for best recommendation of care.
  • Coordinator/LPN refers and triages patients to other sources of treatment when appropriate.
  • Coordinator/LPN answers all inquiry calls regarding treatment and the facility, presents treatment options, documents calls and sets appointments for callers seeking treatment.
  • Coordinator/LPN works with the business office to ensure smooth transition into the admission process.
Qualifications:
Requirements:
  • Graduate from an accredited program of professional nursing training or relevant state licensure required.
  • Current unencumbered license to practice by the State Board of Nursing
  • CPR Certification and Crisis Prevention Training (CPI) within 30 days of employment and prior to any patient contact
Preferred Experience:
  • Previous experience in a psychiatric health care facility, with direct experience working with chemical dependency, dual diagnosis, psychiatric and geriatric patients preferred.
  • Experience in patient assessments, family motivations, treatment planning and communication with external review organizations or comparable entities.
