GENERAL SUMMARY/ OVERVIEW STATEMENT:



Reporting directly to the IOM Division Chief for the joint MGH and BWH IOM divisions, the IOM Educator
is part of the leadership group of the IOM technologist teams at the MGH and the BWH.
In this capacity, the IOM Educator provides direct training, oversight of training activities and quality
assurance to IOM technologists at Massachusetts General Hospital and Brigham and Women’s
Hospital. The IOM Educator works jointly with the respective single site Chief Technologists by
assessing policies and practice in both programs with the goal of aligning technologist practice across
both campuses. This person will augment training efforts of Chief Technologists for both new hires as
well as existing team members. This person will lead the development and management of competency
requirements across both programs.



In addition, primary responsibilities include providing coverage of direct supervision of IOM technologist
at either institution together with the respective site Chief IOM Technologists, including troubleshooting
of all modalities of neuromonitoring.



Occasionally, the IOM Educator may perform cases at both institutions and functions at the IOM
Specialist skill level, performing standard and complex intraoperative neurodiagnostic procedures as
described by the MGH and BWH Neuro Intraoperative Monitoring protocols. The technologist provides
troubleshooting and technical interpretation of intraoperative neurologic data during surgery in
collaboration with the monitoring IOM physician to the surgical team and the anesthesiology team.
All IOM technologists are trained in and responsible for the accurate recording and documentation of
SSEP, TcMEP, EMG/triggered EMG, BAEP, and EEG waveforms. In addition, the technologists are
responsible for recording and the documentation of cortical motor/sensory mapping, ECoG, and direct
peripheral nerve stimulation cases. The technologist also assists in the maintenance of equipment,
patient data, and supplies as directed by the IOM leadership team.



The IOM Educator in particular will act as an interactive part of the operating room team, assisting in
duties as appropriate. As MGH and BWH continue to research and apply intraoperative monitoring
methods and techniques to new procedures, the IOM Educator will participate in developing new
protocols and will instruct other technologists in said protocols.

The IOM Educator also displays leadership qualities and is able to identify potential areas of concern and communicate these concerns to the IOM Division Director. This role functions on an independent basis, requiring little support from other staff members.


PRINCIPAL DUTIES AND RESPONSIBILITIES:



Education:
  • Takes a primary role in the training of staff, fellows, technologists, students etc.
  • Participates in training process for newly hired technologists and existing technologists who are learning more complex intraoperative neuromonitoring procedures. Partners with IOM chief technologists from both institutions on this effort
  • Participates in orienting and mentoring new employees in the departments together with the single site Chief Technologists
  • Participates in assessment of competencies for IOM technologists and devises educational plan to help technologists achieve competency requirements
  • Develops quality assurance infrastructure for intraoperative neuromonitoring technologists
  • Evaluates department processes and workflows and appropriately recommends improvements
  • Assesses technical protocols and practices across MGH and BWH programs with the goal of aligning technologist practices
  • Participates in the development and implementation of new clinical and research protocols at either hospital
Supervision:


  • Provides supervision coverage for IOM technologists at either site in the following scenarios:
o As part of the educational process
o In the absence of the single site Chief Technologists
o In collaboration with the single site Chief Technologists
o At the direction of the Division Director
  • Possesses decision making skills and authority at the management level
  • Determines adequacy of IOM equipment


Information management:
  • Participates in the maintenance of IOM databases across the institutions
  • Communicates and collaborates with the IOM Clinical Technology Director with regards to machine maintenance, repairs, and replacements
  • Communicates and collaborates with the IOM Clinical Technology Director with regards to IT protocols
  • Is skilled in programming IOM monitoring templates and regularly teaches other technologists in these skills
  • Participates in the development and assists with the implementation of relevant Epic or other HER protocols including providing training and producing training materials


Practical Intraoperative Neuromonitoring Skills:
  • The IOM Educator is skilled in providing intraoperative monitoring for all surgical cases as requested at MGH and BWH. This technologist works independently requiring very little assistance and is an advanced technologist able to provide and instruct others in the following:
o Continuous on-line technical interpretation to the surgical and NIOM teams throughout the procedure. The technologist also directs the surgeon in application of sterile electrodes and stimulating devices in surgical field.
o Assures appropriate communication with the interpreting neurophysiology physician
o Assures that real-time remote monitoring is provided as appropriate for the case
As part of the clinical workflow, the IOM Educator can provide and instruct others about the following:
  • Informs the patient about the neurodiagnostic procedure and answers questions as appropriate and necessary
  • Abstracts relevant clinical information from the patient's chart, the patient and from the surgeon when necessary
  • Knowledgeable in preparing patients prior to draping for surgery, setting up for all possible appropriate monitoring procedures. Preparation is by applying electrodes and stimulators with the utmost accuracy and security, as necessary for long duration and sensitive surgical procedures
  • Skilled in advanced troubleshooting
  • While monitoring, meticulously documents all surgical and related procedures and communications with the surgical team
  • Prepares a preliminary technical report describing the recording throughout the procedure.
  • Assists with technical and physician billing


Knowledge required:
  • Relevant anatomy, neurophysiology
  • Comprehensive knowledge of all monitoring modalities
  • Surgical procedures and the neurological structures at risk
  • Anesthetic protocols and their effect on waveforms
  • Policies and procedures of both the hospitals
  • Computer networking and template programming
  • Relevant monitoring software and hardware


Budget Responsibility:
  • Assist with analysis of competitive position of technologist salaries in the labor market
  • Participates in budgeting of the IOM department supply usage and waste
  • Participates in monitoring premium and purchased labor hours (agency) to stay within budget constraints
  • Participates in managing expenditures for special and / or research projects
  • Provides input and makes decisions regarding work processes based upon existing policy interpretation and industry guidelines
  • Participates in the development, selection, and implementation of software and procedures needed to provide reliable monitoring
  • Participates in hiring new technologists and planning of the future of the department

SUPERVISORY RESPONSIBILITY:
Direct: Functions as IOM Chief Tech at either hospital as required
Indirect: Function as IOM Chief Tech in the absence of the single site IOM Chief Technologist


SKILLS & COMPETENCIES REQUIRED:



All skills and competencies of IOM Technologist and IOM Technologist II plus:
  • Analytic ability to evaluate the patient history and determine the appropriate testing procedures, determine whether equipment is working properly, and troubleshoot any malfunctions that may occur
  • Excellent interpersonal skills to lead and develop a team of technologists effectively as well as interacting with other Hospital employees and patients
  • Excellent organizational skills to manage multiple tasks and complete them in a timely manner
  • Excellent communication skills to clearly and concisely communicate verbally and in writing with peers, superiors, payers, physicians, ancillary departments, etc.
  • Ability to adapt to urgent deadlines or rapidly changing priorities and maintain composure in high stress situations at all times
  • Basic understanding of computer programming and networking
  • Ability to be self-sufficient to perform challenging and daily tasks/functions

  • ABRET R. EEG T. or BS (or higher) in a health science discipline

EDUCATION:

Certificate: Graduate of an accredited electroneurodiagnostic training program and/or equivalent degree (i.e. BS degree (or higher) in a health science discipline

EXPERIENCE:
Required: The technologist must be registered by ABRET as CINM. An active BLS certification is required. In addition, the technologist must have a minimum of three year’s experience monitoring procedures in the operating room under the supervision of a qualified physician neurophysiologist


WORKING CONDITIONS:
  • Work takes place in the OR, in which a sterile environment must be maintained.
  • OR temperatures are typically cooler than normal office temperatures.
  • OR rooms are separated and may require walking between rooms.
  • Computer systems may weigh 80-100+ and are mounted on carts, which will need to be pushed to and from operating rooms.
  • OR cases by nature may be long in duration. Position may require sitting or standing in a room for multiple hours.
  • Risks associated with the handling of needles and sharp equipment.
  • Risks associated with OR environment including contact with bodily fluids and infectious disease.
