Responsible for completing telephone admissions process (for People Inc. programs); organizing and maintaining the collection and storage of clinical information related to individuals requesting services. Obtains necessary documents for service requested. Responsible for processing the DDP-1s to ensure for agency billing. Keeps information updated in People Inc.’s systems for utilization and support to other departments.

ESSENTIAL FUNCTIONS/RESPONSIBILITIES:

  • Completes telephone admission with all individuals interested in obtaining services at People Inc. and completes tracking of calls.
  • Monitors individual admission process including but not limited to forwarding necessary paperwork to families, Care Coordinators and programs to complete as necessary.
  • Creates individual -file to include all required agency paperwork. Contacts the Care Coordinator/program of any outstanding paperwork to be reviewed with or signed by the individual.
  • Responsible for adding information to the appropriate data base for individuals coming through Admissions for programs.
  • Maintains documentation for member files, and coordinates purging of files and records storage with Corporate Compliance Department personnel
  • Monitors OnBase for DDP-1 changes and uploading information to CHOICES utilizing OnBase. Ensures DDP-1s are processed in Choices and maintains tracking of process.
  • Enrolls and updates Individual Data in Therap, Client Track and other agency databases for all applicants based on department requirements, including enrollments into People Inc., changes in program status and residential moves. Does proper notifications to other departments, including support department (ie. Business office).
  • Assist agency staff with Choices user access forms and is a Choices Champion as delegated.
  • Supports Family Education and Training (FET), Excalibur or other programs to ensure participation, including, but not limited to enrollment paperwork, scheduling, participation and maintaining files as needed and ensures all documentation is in Medicaid Compliance.
  • Attends Department and program staff meetings, Agency Intake Committee and trainings.
  • Responsible for adding information to the appropriate database for individuals coming through Admissions for programs.
  • Participates in outreach and community events on behalf of People Inc.
  • Attends trainings as required by OPWDD/DOH and trainings deemed necessary to fulfill job requirements.
  • Supervises students, volunteers, aides or other individuals as assigned.
  • Maintains confidentiality and process, maintains and utilizes potentially sensitive information in accordance with agency and departmental policies and individual’s Bill of Rights.
  • Completes weekly schedules accurately and timely in accordance with department policy.
  • Complies with all agency policies and procedures.
  • Participate in supervision.
  • Other duties as assigned.

MINIMUM QUALIFICATIONS:

  • Bachelors of Science/Arts degree with 2 years relevant experience, OR
  • Master’s Degree with 1 year relevant experience
  • Working knowledge of developmental disabilities, special needs and awareness of community services and programs.
  • Ability to organize, plan and manage caseloads.
  • Must have sound judgment, flexibility, dependability and good writing skills.
  • NYS Driver’s License that meets agency policy.
  • Ability to handle multiple tasks simultaneously.
  • Ability to meet physical requirements of the position when performing the following: standing, walking, sitting, bending, stooping, squatting, kneeling and climbing
