POSITION SUMMARY

The Field Nurse Supervisor (Community Health Nurse) is salaried employee who will provide healthcare education, assistance and management to older adults to help them age-in-place in the community. In this rewarding position, she/he will participate in a multi-agency interdisciplinary team promoting healthy, successful aging for older adults within Plainview and Old Bethpage, through a state-funded program. The Community Health Nurse will maintain a caseload of healthcare management clients, and also provide general health education and assistants to older adults who reside in these communities.

QUALIFICATIONS (Must Haves)

· Currently licensed Registered Nurse in good standing, in the State in which services are provided.

· Graduate of an accredited school of nursing.

· At least two-years of experience in an acute care setting, one of which is in a home healthcare setting is preferred.

· Current CPR certification and must maintain certification.

· Maintain nursing license by obtaining the necessary contact hours and competency.

· Possess effective communication skills and be able to follow instructions to complete work as scheduled.

· Active and current driver's license and have a dependable insured automobile. Must be comfortable with significant daily driving to and from client residences.

· Possess excellent people and customer service skills with excellent communication skills, both oral and written.

· Reliable and have a positive attitude, which fosters a cooperative and positive atmosphere.

· Neat in appearance.

RESPONSIBILITIES

· The following duties and responsibilities reflect the essential functions for this job, but do not restrict the tasks that may be assigned. The Director of Nursing or Regional Director of Operations may assign or reassign duties and responsibilities to this job at any time due to reasonable accommodation or for other reasons.

· Performs the essential activities including assessment, planning, implementation, coordination, monitoring, evaluation, outcomes, and general interventions across case management practice and processes, and makes appropriate referrals to other disciplines.

· Performs initial evaluation and assessment of client needs and contacts physician _AS NEEDED

· Establishes the Care Plan with team and coordinates, implements appropriate alternatives, and makes a determination of expected client outcomes and goals.

· Updates and revises the Care Plan as necessary.

· Makes appropriate referrals for therapies and medical equipment/supplies based on client needs.

· Observes client for changes in condition and obtains orders for changes to the Care Plan, documents all observations and interventions in the clinical record in a timely manner.

· Develops and keeps current orders for medications and treat approval as appropriate.

· Uses appropriate preventive, therapeutic, and rehabilitative nursing procedures and keeps client's physician, family and the Nursing Supervisor informed of care being provided.

· Assists and instructs client and family members in nursing care required to promote client's health and safety.

· Documents nursing observations, interventions and evaluations of the care being provided in accordance with agency policy.

· When assigned, supervises ancillary personnel.

· Participates in team conferences, as requested, and attends staff in-services programs.

· Thoroughly explains the Company’s mission and value to the client and client’s family members, doctors, providers, etc.

· Explains role and services of the Company and the aging-in-place program, describes client’s rights, and provides contact information for complaints.

· Researches client information regarding diagnosis and treatment options as necessary and educates family as to disease process, self-care techniques and prevention.

·Researches medical history, care and/or equipment costs, community and/or government programs and other information that would contribute to or enhance cost-effective quality care for the client.

·Responsible for accurate, timely, and ongoing evaluation and reporting of client’s needs, services, & outcomes.

·Monitors and documents progress of client issues, goals, and outcomes; documents findings and appropriate cost effective recommendations

Acts as liaison between client, family, physician, providers and facilities.

·Documents assessments, progress, re-evaluations, and other pertinent information in the client’s documentation system.

·Identifies problems and sets goals.

. Performs other duties as required.

·Coordinate multiple tasks simultaneously.

·Exercises standard precautions.

·Actively involved with agency Quality Improvement.

·Collect, interpret, and/or analyze complex data and information .

·Understand and respond to a diverse population.

·Sit and drive for long periods of time.

· Physically capable of walking, climbing stairs

·Required to be able to communicate electronically.

SCHEDULE RESPONSIBILITIES

· Full time, salaried position, primarily during the normal business day.

Job Type: Full-time

Pay: $90,000.00 - $100,000.00 per year
