Job title: Activity Assistant

Location: Crest View Nursing & Rehabilitation Center - New Lisbon, WI

Schedule:

• Full-time, 75 hours per pay period; Monday - Friday, 9:00am-5:00pm

Weekend rotation: Every third weekend

Holiday rotation: Holiday rotation required

Position Summary:

Develops and administers a comprehensive activity program to meet the ends, interests, and capabilities of the long-term skilled nursing-unit population. Assumes the responsibility to develop and administer a well-rounded hospital-wide social program schedule.

Position Responsibilities:

• Assists in organizing, implementing, leading and monitoring recreation programing to meet the assessed needs, interests, and capabilities of the residents.
• Records resident involvement in recreation.
• Attends department meetings, committee meetings and in-services.
• Works with staff, family/caregiver and residents regarding resident interests, needs, and limitations, as well as the availability of resources.
• Assures that all activities conform to the criteria of federal, state, and local accreditation and licensing agencies or organizations.
• Assists in development and distribution of recreation calendar.
• Coordinates programs with other departments in the hospital and interfaces with all personnel as appropriate.
• Uses community resources in the development of programs.
• Assists with the development and provision of training and orientation programs for students and volunteers.
• CNA certified employees will attend a minimum of 12 hours of in-service as required. Assist in CNA duties, especially as related to the recreation department, as appropriate.

Position Requirements:

Minimum Education

High school diploma or equivalent degree preferred.

Required Skills

Understands human behavior and modification.

Is tactful, pleasant, and friendly approach in dealing with patients, staff, guests, and individuals within the community.

Is capable of identifying signs of unfavorable symptoms such as fatigue and irritability.

Has sound judgment and the capability to respond to unusual circumstances.

Basic computer skills preferred.

CNA certification preferred.
