LPN Team Lead

RESPONSIBILITIES:

• Lead the team on your assigned unit, assuring completion of all assigned resident care needs
• Provide direct "hands-on" patient care, including passing medications and performing treatments
• Oversee and guide CNA's work to ensure quality resident care and completeness
• Assists in the admission, transfer & discharge of residents
• Communicate closely with the Supervisor/Manager in terms of resident problems, progress, and care plans
• Direct and demonstrate individualized supportive care to the residents and their families while working cooperatively as an integral member of the multidisciplinary team

ATTRIBUTES OF SUCCESSFUL LPNs:

• Current Licensed Professional Nurse license in the State of New York
• Good organizational and communication skills
• Respect for the privacy, dignity, and confidentiality of all residents
• Prior experience in long-term care is helpful but not necessary
