Position: Therapist
Department: Behavioral Health
Reports To: Behavioral Health Clinical Manager
Position Status: Full-time, Exempt - Belmont Cragin (3 days), Wicker Park (2 days) per week

Position Summary
As part of the Behavioral Health Team, the Therapist works in an integrated primary care setting as a regular team member and delivers long term outpatient trauma-informed therapy services to clients, family members and significant others for a wide range of symptoms related to anxiety, mood disorders, trauma, and behavioral issues. This position will provide therapeutic, evidence-based services to children, adolescents, and adults to promote behavior change, reduce risk behavior and improve general overall health.

Duties and Responsibilities
  • Conduct clinical assessments of clients struggling with behavioral issues through use of clinical interviews and standardized measures.
  • Determine appropriate diagnosis and treatment regime dependent on the client’s needs, culture, home environment and preferred method of therapeutic intervention.
  • Provide evidence-based psychotherapy services, including individual, family and group services, to a caseload of children, adolescents, and adults.
  • Conduct suicide and homicide risk assessments, as indicated.
  • Contact collaborative resources for information related to client care, and coordinate with outside community resources, as needed.
  • Be available for client consultation to primary care staff including PCPs and BHCs by phone or EHR.
  • Identify, review, and modify client educational materials for literacy level and cultural appropriateness (under appropriate supervision).
  • Facilitate and assist with referrals to community resources as needed.
  • Complete clinical notes and other necessary paperwork in a timely manner to comply with all administrative regulations. Complete visit notes within 72 business hours of visit completion.
  • Support program evaluation reports and Quality Improvement measures, as needed.
  • Attend required meetings regularly, including weekly supervision, monthly group supervision, and BH, Site, and Provider meetings.
  • Other duties as assigned.
Required Skills or Abilities
  • Excellent interpersonal skills and ability to work collaboratively with primary care staff, clients, and other behavioral health clinicians.
  • Highly skilled in providing behavioral health services with experience working with a wide range of ages and presenting issues.
  • Knowledge and interest in providing trauma-informed behavioral interventions.
  • Maintain their own schedule to meet client demand.
  • Proficient in the use of computers and Microsoft Office.
  • Proficient in the use of an electronic health record.
  • Comfortable with utilizing technology at all points of the day, including telehealth software, video communication, and internal communication tools.

Physical Demands
  • Must be able to remain in a stationary position 75% of the time.
  • Must be able to move around the clinic site 25% of the time.
  • Constantly operates a computer, computer printer, copy machine, and telephone.
  • Occasionally positions self to maintain exertion of physical strength to move objects of 10 pounds from one level to another.
  • This position will work from two different locations so therapist must be able to transport to each site.

Required Knowledge, Exper
ience, or Licensure/Registration
  • Master’s degree in Counseling, Social Work or Psychology.
  • LCSW, LCPC or Psy.D./Ph.D with active licensure preferred.
  • Must possess all licenses, registrations, certifications, and accreditations required by law to render health care services in the state of Illinois.
  • Must maintain full credentials and privileges to practice with PrimeCare.
  • Minimum of two years of clinical experience as a behavioral health provider in a health care setting required.
  • Work schedule-at least 2 evenings or 1 evening and Saturday per week.
  • Previous experience in a medical setting preferred.
  • Strongly preferred bilingual in English/Spanish.
