Summit Home Care is searching for a dedicated FT Speech Therapist (ST/SLP) to join our growing team! We are dedicated to providing our patients with the highest quality home health care services available and doing so with world-class customer service. This is a great opportunity to utilize your SLP skills and provide quality one-on-one care with a focus on the individual patient and positive outcomes. As a Speech Therapist, you will operate under the supervision of the Clinical Management Team while providing direct care to patients in the at-home setting.

Position: ST/SLP Hours / Shift Schedule – FULL TIME Basis

EDUCATION - The minimum level of education desired for candidates in this position is an ST/SLP License.

Qualifications:

  • ST/SLP License required
  • Previous experience in speech pathology preferred
  • Ability to build rapport with patients and SNF staff
  • Excellent written and verbal communication skills
  • Positive and patient demeanor

Responsibilities:

· Administers speech therapy programs utilizing specialized therapeutic techniques and/or equipment.

· Conducts appropriate evaluations, assesses the SNF environment

· Confers with referring physicians and other organization personnel to ensure coordinated and comprehensive care contributes to the total plan of care.

· Counseling individuals and families on ways to cope with swallowing and communication disorders

· Creating and carrying out individualized treatment plans addressing specific functional needs

· Evaluating levels of swallowing, speech, and language difficulty

· Helping individuals improve sentence structure and vocabulary used in written and oral language

· Identifies equipment needs relative to speech/language function

· Identifies functional speech defects and establishes a plan of care to improve the patient's function

· Identifying treatment options

· Maintains clinical competency in speech-language pathology practice and theory.

· Prepares and submits notes per organization policy.

· Recommends communication devices/aids as indicated.

· Teaching patients how to make sounds while improving their voices and maintaining fluency

· Working with adults and children in developing and strengthening muscles used to swallow
