We have openings for FT entry-level case managers to work with our elderly and disabled clients, helping them access services and resources to remain living in their own home. We manage several state programs so there is lots of paperwork and state rules to follow, but the reward of helping people get the help they need, is worth it for the right candidate.

ACM Care is a local growing social service agency working with senior citizens. Full Time Social Services position working with Senior Citizens in the community, nursing homes, and hospitals; helping our clients obtain quality services in the least restrictive environment as possible. This position involves completing comprehensive assessments of needs, developing community-based care plans for individual clients and their families, providing referrals and support services, and following up. Requires the ability to form working relationships with all our local vendors. Position also includes lots of paperwork and state forms!!!

Job Summary: Responsible for providing case management services to frail elderly residing in Chicago Illinois area to assure the delivery of quality care and an optimal lifestyle to the client. Maintain online and PC-based database programs on case management clients. Participate in Outreach and Networking activities as needed.

Job Type: Full-time

Pay: $40,000.00 - $45,000.00 per year
