JOB SUMMARY: To evaluate and develop a system of carrying out the goals and objectives of consumers to develop skills and abilities based on individual strengths and needs.

Essential Duties AND Responsibilities include the following.
Other duties may be assigned,

· Evaluates the capabilities and disabilities of consumers to develop individualized valued outcomes. Prepares valued outcomes following OPWDD and other agency and professional guidelines, and in accordance with consumers’ treatment team meeting.

· Coordinates team recommendations with Direct Service Providers and professional staff. Prepares a variety of administrative controls such as waiver billing and valued outcome data collection for use by Direct Service Providers.

· Ensure the Direct Service Providers complete daily logs and tracking.

· Completes written monthly reviews of consumers’ progress towards valued outcome progress. Revises valued outcomes as needed to reflect consumer progress and skill levels and ensures continuity of revisions with treatment team.

· Participates in individual consumer case reviews, attending initial 30-day, semi-annual and annual reviews. Writes and presents reports of progress and recommendations for valued outcome revisions based upon documented consumer performance. Coordinates program to ensure continuity of care and implementation of team recommendations with residential staff and Service Coordination.

Education/Experience:

Associate’s Degree and experience with physically and/or multiply disabled consumers, or equivalent combination of education and experience.

Job Type: Full-time

Pay: $20.00 - $22.00 per hour
