Summary
Responsible for overseeing all health care services to Individuals residing in the medical respite house. Must comply with applicable regulatory requirements and standards in the performance of nursing duties.
Schedule: Sunday 11am - 7pm, Monday - Thursday 10am - 6pm (40 Hours)
Essential Job Functions
  • Responsible for all health care services provided to Individuals residing in the medical respite home.
  • Provides direct nursing care, including the administration of medications and treatments prescribed by Individual’s PCP.
  • Provides periodic audits of nursing documentation to ensure compliance with all regulatory requirements.
  • Coordinates and collaborates with licensed clinicians regarding medical issues, decline in status, and change in clinical needs.
  • Coordinates individual’s intake with house manager and LPNs to ensure that all medical needs are met.
  • Serves as a member of the Professional Interdisciplinary Team for each Individual and provides continuing evaluation and review of everyone’s needs and status.
  • Reviews and approves all medication and or medical needs of everyone before entering the home.
  • Collaborates with individuals care providers around medical and nursing supports during respite stay.
  • Completes all required nursing documentation including monthly, quarterly and annual nursing assessments for each Individual.
  • Advises the house staff including LPNs, House Manager, and the Director of Family and Community Supports of any changing medical needs or decline in condition of the individual during respite stay.
  • Coordinates the medical and health related elements of the referral and intake process for incoming individuals in collaboration with the house manager and the LPN’s.
  • Creates nursing protocols and care plans based on individuals specific support needs. Monitors delivery and implementation of all clinical and nursing services.
  • Reviews all incident reports and makes recommendations and/or follow up with appropriate parties.
  • Oversees the handling of medical emergencies.
  • Provides crisis management; assures that human rights, safety and respect for Individuals are safeguarded in all interventions and interactions.
  • Provides training to program staff, as appropriate and applicable in medical/health-related topics, including but not limited to; Universal Precautions for New Employee Orientation and individual’s specialized medical protocols.
  • Works with the Director of Family and Community Supports in the development, revision and enforcement of all policies and procedures related to nursing services, including medication and treatment administration.
Experience & Education
  • Registered Nurse, registered with the MA Board of Registration in Nursing Required.
  • Bachelor’s Degree in Nursing preferred.
  • 1 year experience in a community health setting.
  • 3 years’ experience working with individuals with developmental disabilities.
  • Strong written, verbal, and interpersonal communication skills.
  • Proven ability to document medical protocols and medication administration practices.
