Duties and Responsibilities:

• Provide full range of case management services to IOP consumers as follows: Referrals, linkage, and advocacy for housing, financial, medical, educational, vocational, legal, spiritual, and other needed/indicated community resources and services in support of consumer recovery and community integration. Case service coordination on behalf of consumers with other behavior health, medical, housing, family, and/or hospital and screening service providers to ensure holistic support of consumers' bio-psycho-social-cultural-spiritual needs and related goals.
• Provide case management assessment, planning, and direct services to consumers to enable access to and benefit from required/indicated community services and resources in support of consumer recovery and community integration goals via in-person, phone, or video contact as appropriate.
• Attend and participate in IOP group sessions as available to familiarize self with active IOP consumers and orient consumers to available case management services.
• Provide collateral case management contacts on behalf of consumers for areas described in #1 above.
• Assist consumers with coordinating resources and services described in #1 above.
• Coordinate treatment, services, and case management needs with other agency services and departments.
• Assist consumers with navigating and coordinating transportation and access to IOP resources and services, as well as services described in #1 above.
• Coordinate treatment and case management needs/plans with referral, collaborative, and after-care service providers entities, and family members whenever possible and appropriate.
• Continuously familiarize self and build working relationships with community resources, referring clients as indicated; and take responsibility for professional development as appropriate.
• Meet with supervisor on a regular basis and participate in Program and Center meetings as scheduled. Participate in Center and Program performance improvement activities.
• Complete documentation in accordance with agency policies.
• Other duties as assigned.

REQUIREMENTS:

• Bachelor's Degree (from an accredited University) in Social Work, Psychology, Counseling, or a related behavioral health field
• 2 years supervised experience in working with mentally ill clients
