POSITION DESCRIPTION

The Program Coordinator will coordinate the activities, schedules, and safety plans at both residences. The Program Coordinator will work out of both houses spending at least two days at each house each week. The duties are as follows:

· Oversee in-house resident activities and groups including completing monthly activity schedule by the 15th of each month for the following month. Keep Announcement Boards in both houses up to date.

· Oversee volunteer base including screening, managing, scheduling, and maintaining relationships with all SLSD volunteers and interns.

· Conduct an annual audit every June of all resident files to ensure all necessary documentation is present. Will provide summary report of annual audit to Executive Director.

· Plan, document and execute two emergency planning drills a year.

· Monitors food, paper supplies, and office & emergency supplies to ensure both homes are always properly stocked.

· Research and keep current lists of referrals in each house for employees to utilize the resources for residents. This should include medical and dental services, lists of current halfway houses, shelters, other social service needs, transportation providers, Social Security, and other government services referrals.

· Involve and motivate the other staff members to participate in activities and events for the benefit of residents or other fundraising events.

· Monitors documentation to ensure it is being completed in a timely manner and appropriately. Print out progress notes by the 3rd of each month for MHA to sign and file in chart by 10th of each month. Ensure all resident files are up-to-date and organized.

· Monitors Repair Log daily. Make appointments and arrange to be there or have staff available for follow-through with repairs, and/or obtaining bids for work at both homes.

· Develop a monthly meal plan and shopping/grocery list based on the menu. Staff members can shop but the Director of Operations will be used as a backup to ensure the shopping is completed. Monitor and utilize food donations.

· Develop monthly bathroom, KP and chores schedule for both houses by the 25th of each month.

· Work with residents to also provide meals that are appealing to them as well while keeping in mind SLSD costs and food appropriateness/accessibility. Menu must be completed and emailed to the Executive Director by the 25th of each.

· Develop a monthly staff schedule by the 20th of each month and ensure all sick calls are covered.

· Assist DOO with assessment/intake process for potential residents including monitoring incoming referral forms, contacting potential residents, and conducting an assessment.

EDUCATION & EXPERIENCE

The Program Coordinator two years of working with individuals that are living with mental illness as well as two years overseeing projects and/or programs. A Bachelor’s degree in Social Work, Counseling, Psychology, Sociology or related field is preferred.

REQUIRED KNOWLEDGE, SKILLS, & ABILITIES

· Ability to read and interpret documents and procedure manuals.

· Must be able to write routine reports and correspondence.

· Ability to apply common sense understanding to carry out instructions furnished in written, oral or diagram form.

· Ability to add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions and decimals. Ability to use a calculator a must.

· Demonstrates attention to detail.

· Ability to cultivate and maintain cohesive working relationships with coworkers.

· Works well in group problem solving situations.

· Speaks clearly and persuasively in positive or negative situations; listens and gets clarification; responds well to questions.

· Writes clearly and informatively; edits work for spelling and grammar; able to read and interpret written information.

· Must be able to operate a computer, related equipment and software.

· Ability to manage time and tasks to meet strict deadlines while maintaining quality of work.

· Demonstrate proficiency with Microsoft Office Suite of products (Outlook, Word, Excel)

Requires good hand-eye coordination, arm, hand, and finger dexterity, including ability to grasp, and visual acuity to use a keyboard. The employee frequently is required to sit for long periods of time, reach with hands and arms, talk and hear. Ability to operate a motor vehicle in order to visit other sites, and run errands required.

WORK ENVIRONMENT

The workspace for the Program Coordinator is located in a heavy traffic area. Frequent interruptions from fellow staff members and residents is expected. The sound level is generally low to moderate. Frequent local travel to run errands and perform essential functions of the job is to be expected. The Program Coordinator is required to take occasional trips to attend training, department meetings, and attend agency events.

· All employees must pass a State and Federal Live Scan background check before the start of employment.
