Concordia Visiting Nurses is a Medicare-certified, state licensed, Medicaid approved home health agency operating in nine Western Pennsylvania counties. For well over a decade, Concordia Visiting Nurses has provided high-quality skilled nursing, palliative care, and a variety of therapy services to our patients. Our goal is to deliver quality home health services, in good faith, to those who entrust themselves to our care and to minister to the unique needs of each individual with respect, dignity, and compassion. Concordia Visiting Nurses is seeking a part-time Physical Therapist to serve our patients living in the South Hills Service Area.
POSITION SUMMARY
The Physical Therapist is a licensed professional (LPT) responsible for evaluating, planning and implementing physical therapy service to patients in their home or current residence as prescribed by the attending physician. The therapist works as a member of an interdisciplinary rehabilitation team.
QUALIFICATIONS
Graduate of a state approved school of physical therapy accredited by the American Physical Therapy Association.
Minimum of 6 months acute care or rehabilitation center experience. Outpatient therapy experience may be considered if practice included medical/surgical patients.
Licensed as a Physical Therapist in the Commonwealth of Pennsylvania
Reliable, independent means of transportation to make home visits. Drivers must have a valid driver’s license and drive a car with insurance limits as established in personnel policies.
