$75.00 - $80.00 per hour - Private duty caregiver, Benefits at 30+ hrs/week

The Physical Therapist directs, supervises, evaluates, and provides physical therapy services to patients in the home as prescribed by the attending physician. Each Physical Therapist instructs teaches and supervises patients, their families, and other health team personnel regarding physical therapy procedures as related to the patient's physical therapy programs. We are looking for a Physical Therapist for home health care. Our team is dedicated, compassionate people with a heart of gold for the patients and their families who guide them through life's final journey. The Ideal candidate would be a professional and a caregiver to our patients and families, having excellent communication skills, the ability to listen and discuss matters of daily living for the patient and family, as well as make decisions in the best interest of our patients in accordance with the plan of care, and be able to explain important issues and procedures with the family. The candidate will be trained to support and deliver quality care.

Ideal Candidates will:

• Possess strong communication and interpersonal skills.
• Promote personal safety and a safe environment for clients.
• Maintain compliance with agency policy, procedures, and job requirements.
• Home Health experience is preferred but not required.

Qualifications:

• Graduate of a curriculum of Physical Therapy approved by the council of medical education of the AMA in collaboration with the American Physical Therapy Association.
• Registered, licensed, or certified by the State Board of Medical Examiners of the state in which the physical therapist resides and practices.
• One (1) year of clinical experience is preferred.
