Job description

We are looking for a Per-Diem Social Worker to conduct monthly visits to nursing home dwelling clients currently under guardianship and/or conservatorship with our agency. Visits are vital to clients emotional and psychosocial wellbeing and serve as a support to the agency in decision making and action planning. Ideal candidates will be licensed at an LSW level and will have at least one (1) year of experience within a healthcare setting.

Visits can be conducted on evening and weekends. Case-load for Per-diem social workers averages Ten (10) clients who require visits every 4-6 weeks.

Mileage and Expense Reimbursement weekly

Candidates will act as an advocate and help clients foster their capacities and coping mechanisms in order to improve and better their daily lives and experiences, within specified standards and timescales.

Responsibilities

• Monitor and evaluate clients’ progress and modify treatment plans accordingly
• Maintain accurate records and report on clients’ status
• Act as a key-worker and cooperate with multidisciplinary teams
• Conduct visits to clients in nursing facility and hospital settings to assess emotional wellbeing, appropriateness of placement and care plan changes.

Skills

• Proven work experience as a social worker
• Working knowledge of social theories and practices
• Social perceptiveness and empathy
• Ability to build and maintain professional helping relationships
• Ability to relate and communicate with diverse population and groups
• Resilience along with ability to assess situations
• Willing to submit to background check/CORI
• BS degree in social work (BSW), psychology or sociology
• Licensure at the LSW Level or above or able to obtain licensure within six (6) months of hire.
• Must have a valid Drivers License and reliable transportation
