Our Mobile Van serves those in the Peak Vista community who may otherwise not be able to access our services. This includes those in our community experiencing homelessness as well as those experiencing other barriers to optimal health and wellness, including but not limited to transportation barriers, food insecurity, employment and education barriers, and others. The Mobile Van team can provide scheduled primary care, as well as unanticipated walk-in services, anywhere. We practice in a team-based care model working closely with behavioral health, dental services, and care management. There are no inpatient expectations. We provide back up on call services to our after-hours nurse triage on a rolling basis shared by all providers.

Our ideal candidate has 2+ years of experience practicing care or mobile setting seeing all ages, with an emphasis on complex patient care and understanding of social drivers of health; able to utilize a template based EMR (NextGen); has an unrestricted Colorado license, prefer an active DEA, and is comfortable seeing all ages, and averages 20 patients per day. Must have a strong desire to make a difference in the community. Must embrace a team-oriented approach.

  • Responsible for providing healthcare in our surrounding communities and assisting in the planning and implementation of mobile care opportunities
  • Work closely with the Mobile Services team, community partners, community health workers and community leadership to ensure Peak Vista's Mobile Services programming is impactful and meaningful, while functioning efficiently.
  • Participate in all aspects of mobile health services where needed, including, but not limited to, community events, street outreach, and in patients' place of residence; provide training on various public health topics.
  • High energy and strong physical condition, including but not limited to the ability to hike distances over 5 kilometers and lift at least 30 lbs. over the shoulders.
  • Able to practice with cultural sensitivity and understanding, embracing the tenets of justice, equity, diversity, and inclusion
  • As the Mobile Services Team and its activities are dynamic and constantly evolving, candidate must be flexible and adaptable to different environments, including patient care in an established Peak Vista Health Center, when needed.
  • Capable of a flexible work schedule, with occasional 10-hour days and rare non-weekday shifts.
Peak Vista Community Health Centers is a non-profit healthcare organization whose mission is to provide exceptional healthcare to people facing access barriers through clinical programs and education. We provide primary care services (medical, dental and behavioral healthcare) through 24 outpatient health centers in El Paso, Teller, Lincoln, Adams and Kit Carson counties. Our organization has over 800 employees and serves over 89,000 patients in the Pikes Peak and East Central Plains region.

Federal and State funds are available for loan repayment for eligible candidates. Peak Vista offers a full benefits package including medical, dental, vision, life, optional long term and short term disability and a 403b retirement fund. Additionally, because Peak Vista is a FQHC (Federally Qualified Health Center), the provider's malpractice insurance is covered through FTCA (Federal Tort Claims Act) government funds.

Compensation (Pay): $117,374.40 to $144,355.71 based on experience.
