This position is located at the SC Dept. of Mental Health at Santee-Wateree Community Mental Health Center (CMHC) at the ROADS OF INDEPENDENCE (ROI) Program located in both Sumter and Kershaw Counties.

Under general supervision, provides education, outreach, advanced counseling and emergency services in the complex areas of mental health and/or substance abuse for youth, young adults, and families within the Center’s community mental health program as part of the ROI Healthy Transitions program in Sumter, Kershaw, and Lee counties.

  • Executes specialized programs and services as directed.
  • Provides an array of evidence-based treatment and community mental health services in a culturally competent manner. Clinical services to be performed as privileged, will include, but not limited to assessment, crisis intervention, individual, group, and family therapy.
  • Participates in treatment team/staffing and administrative meetings as designated. Formulates plans of care. Maintains accurate and timely medical records. Adheres to DMH, Corporate Compliance, Fire and Safety, CARF, QA and HIPAA standards.
  • Participates in interdisciplinary and transition team meetings in a consultative manner on issues related to patients in regards to treatment planning and coordination of care. Participates in other designated meetings to offer appropriate consultations, referrals, and mental health services as needed.

Provides case management and ancillary services to youth, young adults and their families in coordination with the local community stakeholders, DMH, and other youth serving agencies in order to facilitate the total treatment of assigned patients.

  • Participates and assists with planning/coordination of community outreach events. Provides education/training/consultation to schools and community partners as requested/directed. Provides in-service training workshops and expertise on culturally responsive topics intersecting the youth and young adult population, i.e., LBGTQ and MH issues.
  • Active participation in data collection and assimilation for reporting outcomes as required.
  • Performs other related duties as required and assigned by the supervisor. Travel throughout the Santee-Wateree CMHC catchment area as directed.

Minimum and Additional Requirements

State Min. T&E – A master’s degree.

Valid driver's license required.

Preferred Qualifications

Prefer candidates licensed as LPC, LMSW, LISW, LMFT, and/or CAC-I/II Certification.
Experience working with children, youth, young adults and families with mental health and/or substance abuse disorders in a clinical setting.

Licensure or licensure eligible in specific field of study. Knowledge of and ability to assess for treatment needs, develop complex treatment plans, and provide effective clinical interventions, including crisis intervention and individual, group, and family therapies. Knowledge of casework practices. Ability to organize and maintain a caseload. Knowledge of legal principles and practices. Knowledge of federal, state, and local laws or regulations pertaining to social service programs.
